马蹄肾合并肾细胞癌3例报告

目的:提高马蹄肾合并肾细胞癌的认知水平。方法:报告3例马蹄肾合并肾细胞癌的临床表现、诊治过程及影象学特点。3例患者中,2例存在反复发生的泌尿系感染,2例合并结石,其中1例有明显的腰部绞痛病史。2例符合根治手术的病例行开放性根治性肾切除术。1例因存在远处转移而仅行生物治疗。结果:2例手术患者随访1年内,未发现肿瘤复发及转移。1例因放弃治疗而于半年内死于肿瘤转移。结论:对于符合根治手术条件的马蹄肾合并肾细胞癌患者,首选的治疗仍为根治性肾切除术。术前充分估计相应的变异血管,细致的腹膜后腔解剖,以及妥善处理峡部,是手术治疗的关键所在。

Renal cell carcinoma in horseshoe kindney:3 cases report

Objective:To investigate the diagnosis and treatment of renal cell carcinoma in horseshoe kidney.Methods:The clinical and image feature of 3 patients of renal cell carcinoma with horseshoe kidney were retrospectively analyzed. Complicated with urinary infection (2 cases), urinary calculi(2 cases) and angina (1 cases) among the 3 cases (2/3 male and 1/3 female). 1 were treated with biotherapy only because of metastasisb and 2 were treated with radical nephrectomy.Results:2 cases of renal cell carcinoma in horseshoe kidney were followed up for 1 year postoperatively and no tumor recurrence or metastasis were found. Another one case died of metastasis for his quit.Conclusions:Radical nephrectomy is the first program in proper cases of renal cell carcinoma in horseshoe kidney. The key of surgical treatment lies in the knowledge of isthmic blood vessel variation and dissecting retroperitoneal space carefully.
