经尿道钬激光同期治疗前列腺增生并发膀胱结石的疗效

目的:探讨钬激光同期治疗前列腺增生并发膀胱结石的临床疗效。方法:分析钬激光同期治疗前列腺增生并发膀胱结石患者47例的临床资料。结果:全部患者手术均获得成功,其中结石平均直径24.6mm(12～40mm),前列腺体积53.9cm3(41～92cm3);钬激光碎石时间10～50min,平均24.5min,手术总体时间103.7(40～160)min;术后平均住院日4.5(3～9)天。术后膀胱结石均完全清除,手术发生膀胱损伤3例,尿外渗3例。无输血和感染病例发生。术后3个月随访,IPSS、QOL评分、最大尿流率和剩余尿量较术前有显著改善,差异有统计学意义(P<0.01)。结论:采用钬激光同期治疗前列腺增生伴膀胱结石是一种安全有效的治疗方法。

Simultaneous transurethral treatment for BPH with bladder stones by holmium laser

Objective:To study the efficacy of simultaneous transurethral treatment for BPH with bladder stones by holmium laser.Methods:Clinical data of 47 cases of patients with BPH and bladder stones were reviewed retrospectively. All these patients were treated with transurethral cystolithotripsy combining transurethral resection by holmium laser.Results:All operations were successful.The mean (range) size of bladder stones was 24.6(12-40)mm and the prostate was 53.9 (41-92) cm3.The mean operation duration of cystolithotripsy was 24.5(10-50)min , The mean operative duration was 103.7 min (40-160)min.The mean duration of postoperative hospitalization was 4.5 (3-9)d .All postoperative patients were stone-free. No patients required a blood transfusion and no infection noted. Three patients had a small capsular perforation or urinous infiltration respectively. All the cases were followed up for 3 months; Qmax ,PVR , IPSS and QOL improved significantly compared with preoperation(P<0.01).Conclusions:Simultaneous transurethral cystolithotripsy and resection of the prostate by holmium appear to be a safe and effective approach for the treatment of bladder stones associated with benign prostate hyperplasia.
