异丙酚复合芬太尼用于无痛人工流产术1162例的临床观察

目的探讨异丙酚复合芬太尼用于人工流产的镇痛效果及安全性。方法选取2008年德阳旗中医院施行异丙酚复合芬太尼人工流产术1162例为观察组,同期同孕周施行人工流产术未用镇痛药物270例为对照组,比较两组手术中、手术后疼痛感与出血情况及复经后月经情况;观察异丙酚复合芬太尼的麻醉诱导时间、恢复时间和手术前、手术中及恢复时的生命体征变化和药物不良反应。结果观察组受术者意识消失时间为(43.65±4.56)s,停药后意识恢复时间为(198.45±23.56)s,下床行走离开手术室时间为(8.76±1.67)min,术中血压、呼吸变化不大,无需特殊处理。与对照组比较,观察组术中无疼痛经历,术后明显疼痛发生率降低,差异有统计学意义(P<0.05);但两组手术时间,手术中出血量,术后疼痛持续时间,阴道流血时间及复经时间比较,差异无统计学意义(P>0.05)。结论异丙酚复合芬太尼用于人工流产镇痛效果满意,安全可靠。

Clinical observation of Propofol anesthesia combined with Fenlanyl for painless induced abortion in 1162 patients

Objective To investigate the efficiency and safety of Propofol anesthesia combined with Fenlanyl for induced abortion.Methods Fenlanyl was injected 20~50 μg intravenously,then 1~1.25 mg/(kg·h)Propofol was injected continuously during the operation in observation group.The patients received induced abortion in the same day without anesthesia in control group.The data of anesthesia induction time,recovery time,vital signs,adverse reactions and the painful feeling,the amount of blood loss,the situation of menstrual recovery were observed in two groups.Results The anesthesia time was(43.65±4.56)seconds,the awakening time was(198.45±23.56)seconds,postoperative activity out of bed time was(8.76±1.67)minutes in observation group.The patients in observation group had no painful experience and postoperative obvious pain incidence decreased that the difference was significant between two groups(P<0.05).The difference in the change of vital signs,operation time,the amount of blood loss,painless duration time after operation,vaginal bleeding time and menstrual recovery time was no significant between two groups(P>0.05).Conclusion Painless induced abortion under Propofol Anesthesia combined with Fenlanyl is the satisfactory and safety abortion surgery.
