不同级别前列腺癌经直肠超声造影参数分析

摘    要:目的分析经直肠超声造影参数在不同级别前列腺癌(PCa)中的价值。方法收集133例临床确诊PCa患者的数据,均经直肠行前列腺超声造影,根据病理Gleason评分将前列腺癌分为5级,采用造影分析软件,比较不同级别前列腺癌的超声造影参数及其差异。结果Gleason评分与造影参数PI、AUC及WIS呈正相关,r分别为0.330、0.328、0.295。并且随着Gleason评分等级增加,血清PSA、造影参数PI、AUC及WIS相应升高。造影参数PI对鉴别1级与4、5级PCa具有较高的特异度,TNP分别为92.0%、96.0%。结论经直肠超声造影参数PI可以反映不同级别PCa的血管灌注信息,行前列腺穿刺活检应结合超声造影参数,在造影参数PI强度较高的区域增加穿刺针数,可以获得更多的Gleason评分高的PCa组织标本。

Abstract:Objective To analysis the value of transcrectal contrast-enhanced ultrasound in different grades of prostate cancer(PCa).Methods Clinical data and imaging data of PCa patients.133 untreated patients underwent transrectal contrast-enhanced ultrasound and prostate biopsy.According to the pathological Gleason score,the PCa was divided into five grades.Contrast analysis software was used to compare the contrast parameters of ultrasound contrast in different grades of prostate cancer.Results Gleason score was positively correlated with contrast ultrasound parameters PI,AUC and WIS(r were 0.330,0.328,0.295,respectively).The PSA,contrast parameters PI,AUC and WIS increase as the Gleason score goes up.PI has a high specificity for the identification of grade 1,grade 4 and 5 PCa,with TNP of 92.0%and 96.0%respectively.Conclusions The transrectal contrast-enhanced parameter PI reflects the vascular perfusion information of different grades of PCa.The prostate biopsy should be combined with ultrasound contrast parameters to increase the number of needles in areas with high PI intensity,and it is expected to obtain more PCa tissue specimens with high Gleason score.
