“4”字体位关节镜下后内侧双通道治疗腘窝囊肿

目的探讨"4"字位关节镜下治疗腘窝囊肿的疗效及优点。方法收集2017年2月～2018年2月在我院采用"4"字体位关节镜治疗的腘窝囊肿患者35例。所有患者住院后采用"4"字体位关节镜后内侧双通道治疗,术后腘窝留置负压引流管,术后48 h内拔除,术后患肢弹力绷带包扎14 d。收集其手术时间,术后第1天疼痛情况,术后14 d疼痛情况,术后复发情况及并发症情况。结果平均手术时间(65.2±8.9)min。术后第1天疼痛VAS评分(2.0±0.5)分,术后14 d疼痛VAS评分(1.0±0.4)分。1例患者弹力绷带包扎1周自行拆除,术后14 d随访发现腘窝肿胀,予穿刺抽出30 mL淡血性液体后,重新弹力绷带包扎14 d后,未再复发,其余患者随访期间内未见囊肿复发。结论 "4"字体位关节镜下双通道治疗腘窝囊肿,创伤小,显露清楚,复发率低。

Posterior medial double-channel arthroscopy with the body position of the Chinese character of "four" in the treatment of popliteal cyst

Objective To explore the efficacy and advantages of arthroscopy with the body position of Chinese character of "four" in the treatment of popliteal cysts.Methods A total of 35 patients with popliteal cysts who were treated with arthroscopy with the body position of Chinese character of "four" in our hospital from February 2017 to February 2018 were collected.After hospitalization,all patients were treated with posterior medial double channel arthroscopy with the body position of Chinese character of "four".After operation,a negative pressure drainage tube was placed in the popliteal space,which was removed within 48 hours after operation.The affected limb was bandaged with elastic bandage for 14 days after operation.The operation time,pain on the first day after surgery,pain on the 14th day after operation,postoperative recurrence and complications were collected.Results The average operation time was(65.2±8.9) min.On the first day after operation,the pain VAS score was(2.0±0.5) points,the pain VAS score was(1.0±0.4) points on 14th day after operation.The elastic bandage was removed by one patient after one week,and during the follow-up visit on the 14th after operation,the popliteal fossa was found swellon.After 30 mL of light bloody liquid was drawn by puncture,the patient was given elastic bandaging for 14 days again,and there was no recurrence.No cyst recurrence was observed in the rest of the patients during follow-up visits.Conclusion Double-channel arthroscopy with the body position of Chinese character of "four" in the treatment of popliteal cysts has small trauma,clear exposure,and low recurrence rate.
