超声造影参数成像技术在眼球后方占位性病变中的应用价值

摘    要:目的运用超声造影参数成像技术评价超声造影在眼球后方占位性病变中的应用价值。方法回顾性分析经超声造影检查的眼球后方占位性病变患者63例,以病理结果为金标准分为恶性组14例,良性组49例。应用SonoLiver造影软件分析超声造影图像,获取时间-强度曲线(TIC)、动态血管模式曲线(DVPC)及定量参数,分析及比较两组间DVPC、TIC曲线特点,比较两组病灶区的到达时间(AT)、上升时间(RT)、达峰时间(TTP)、峰值强度(IMAX)、平均渡越时间(mTT)、上升支斜率(RS)及下降支斜率绝对值(DS)等指标的差异,并行ROC曲线分析获取鉴别诊断的最佳截断值。结果(1)良性组TIC主要表现为快升慢降波,DVPC主要表现为正向波,恶性组TIC主要表现为快升快降波,DVPC主要表现为正负双向波,两组差异均具有统计学意义(P<0.05);(2)两组TIC定量参数mTT、RT、RS、DS差异均具有统计学意义(P<0.05),而IMAX、AT、TTP差异无统计学意义(P>0.05);(3)当RT为11.8,mTT为29.2,RS为115.8,DS为45.4是鉴别眼球后方良恶性肿瘤的最佳截断值,其中RS为115.8鉴别诊断价值最高。结论超声造影参数成像技术可为眼球后方占位性病变的鉴别提供一定的诊断依据。

Abstract:Objective To investigate the value of contrast-enhanced ultrasound parametric imaging in contrast-enhanced ultrasound(CEUS)diagnosis of space-occupying lesions behind the eyeball.Methods The 63 patients with ospace-occupying lesions behind the eyeball who underwent CEUS examination were retrospectively analyzed.These patients were divided into two groups depended on histological diagnosis including 14 cases with malignant tumor and 49 cases with benign tumor.CEUS images were analyzed using Sonoliver software.The time-intensity curve(TIC)and dynamic vascular pattern curve(DVPC)were acquired and the characteristics of the arterial flow infusion of these were analyzed.The related parameters such as time of arrival time(AT),rise time(RT),time to peak(TTP),mean transit time(mTT),slope of rise(RS),the absolute value for slope of down(DS)were also obtained.The differences of two groups were compared.Receiver operating curve analysis was used to acquire the cut-off value of these parameters for differential diagnosis.Results(1)TICs of the benign tumors were mainly fast rise and slow drop while TICs of the malignant tumors were mainly fast rise and fast drop,DVPCs of the benign tumors were mainly positive while DVPCs of the malignant tumors were biphasic;(2)The quantitative parameters RT,mTT,RS and DS of TICs were statistically significant between the two groups(P<0.05),while IMAX,AT and TTP were not(P>0.05).(3)RT=11.8,mTT=29.2,RS=115.8,DS=45.4 were the best cut-off value for differential diagnosis.Among these parameters,RS was 115.8 had the highest diagnostic value.Conclusions The quantitative CEUS parameters can provide effective reference for differential diagnosis of space-occupying lesions behind the eyeball.
