开腹胆道旁路手术与开腹胆管切除术治疗胆管癌患者的临床疗效观察

目的 探讨在胆管癌治疗中采取开腹胆道旁路手术的价值。方法 选取2017年5月～2019年10月我院收治的胆管癌患者100例为研究对象。依据病历单双号随机分为观察组和对照组,每组50例,对照组予以传统开腹胆管切除术,观察组实施开腹胆道旁路手术,对两组患者术中出血量,手术时长,术后肛门排气、下床时长,住院时长及并发症发生情况进行统计并对比,评估疗效、胆功能恢复情况。结果 观察组术中出血量,治疗后TBIL值,术后肛门排气、下床时长及住院时长均优于对照组;同时并发症发生率低于对照组(P0.05)。结论 采取开腹胆道旁路手术治疗胆管癌相比传统开腹胆管切除术,不仅术中出血较少、且术后并发症少、恢复较快,疗效更佳,值得推广。

Clinical observation on effects of open bile duct bypass surgery and open bile duct resection for patients with bile duct cancer

Objective To study the value of open bile duct bypass surgery in the treatment of bile duct cancer.Methods 100 patients with bile duct cancer admitted to our hospital from May 2017 to October 2019 were selected as the research subjects.According to the odd or even number of the medical records,the subjects were randomly divided into two groups,the observation group and the control group,with 50 cases in each group.The control group was treated with traditional open bile duct resection,while the observation group was treated with open bile duct bypass surgery.The intraoperative bleeding,length of operation,postoperative passage gas anus,duration of getting out of bed,length of hospital stay and complications were analyzed and compared to evaluate the efficacy and recovery of bile function.Results The amount of intraoperative bleeding,total bilirubin(TBIL) value,postoperative passage gas anus,duration of getting out of bed,and length of hospital stay after treatment in the observation group were better than those in the control group; The incidence of complications was lower than that in the control group(P<0.05).Conclusion Compared with traditional open bile duct resection,open bile duct bypass surgery for bile duct cancer not only has less intraoperative bleeding,but also has fewer postoperative complications featured with faster recovery and better curative effect,and is worth promoting.
