摘 要: | 目的应用经会阴超声评估子宫切除术后远期女性人群盆底情况。方法选取患者277例,经盆底超声检查,填写PFD相关症状问卷,统计超声表现下盆底功能结构变化情况及PFD相关症状的发生率。结果(1)超声表现上提示PFD共190例(68.59%),其中以膀胱膨出(48.38%)最多见,余下依次为会阴体过度运动、尿道内口开放、膀胱颈移动度增加、穹隆脱垂、直肠膨出、肛提肌陈旧性损伤、肠疝;221例(79.78%)女性出现PFD相关症状,以尿失禁发生率(54.15%)最多,余下依次为便秘、尿频尿急、性交痛、排尿困难、粪失禁。(2)不同术式人群之间PFD发生率无统计学意义(P>0.05)。结论经会阴盆底超声可以全面评估子宫全切术后远期女性人群盆底情况,PFD发病率及其盆底功能障碍相关症状出现情况显著增加。

Abstract: | Objective To female pelvic floor condition after total hysterectomy by transperined pelvic ultrasound.Methods 277 patients were selected.Transperineal pelvic ultrasound was peformed and PFD-related symptom questionnaire was filled-out.The changes of pelvic floor functional and structure and the incidence of PFD-related symptoms were statistically analyzed.Results(1)Ultrasound findings showed a total of 190 cases of PFD(68.59%),of which cystocele(48.38%)was the most common,followed by overactive Perineal body,urethral opening,overactive bladder,Vault prolapse,rectocele,levator ani muscles,enterocele.221 cases(79.78%)had PFD-related symptoms,and the incidence of urinary incontinence(54.15%)was the most,followed by constipation,urinary urgency and frequency,sexual pain,dysuria,and anal incontinence.(2)In these three groups,there was no statistical difference in the incidence of PFD(P>0.05).Conclusions Transperineal pelvic ultrasound can more comprehensively evaluate the structure and function of the pelvic floor after long-term hysterectomy,the incidence of PFD and the PFD-related symptoms were significantly increased. |
