颅脑超声检查母体妊娠期糖尿病的新生儿脑损伤的诊断价值

摘    要:目的通过颅脑超声检查母体妊娠期糖尿病分娩的新生儿,研究新生儿颅脑损伤情况及与妊娠期糖尿病相关性的诊断价值。方法选取我院376例新生儿行颅脑超声检查,母体妊娠期糖尿病分娩的新生儿193例为研究组,母体无高危围产因素分娩的新生儿183例为对照组,分析各妊娠孕周分娩的新生儿颅脑损伤情况,并研究了与母体妊娠期糖尿病的相关性。结果新生儿超声检查颅脑损伤率研究组显著高于对照组,有统计学意义(P<0.001);母体妊娠期糖尿病与颅脑损伤率呈正相关,有统计学意义(P<0.05)。结论颅脑超声是新生儿颅脑检查首选和动态监测的方法,可用于评估母亲妊娠期糖尿病分娩的新生儿颅脑损伤情况,可为有妊娠期糖尿病母体分娩的新生儿做临床早期诊断及干预提供诊断依据。

Abstract:Objective To discuss the value in diagnosis of ultrasonography on newborns whose mothers had gestational diabetes mellitus,and to study the correlation between neonatal brain injury and gestational diabetes.Methods Selected a total of 376 neonates in neonatal intensive care unit undergoing craniocerebral ultrasonography.193 newborns with maternal gestational diabetes mellitus were enrolled in the study group,and 183 neonates without maternal delivery of high-risk perinatal factors were the control group.The characteristics of brain ultrasound images were observed respectively.The perinatal data and brain injury of the two groups were analyzed,and the correlation between gestational diabetes mellitus was studied.Results The neonatal preterm birth study group,which was statistically significant(P<0.001)compared with the control group.Neonatal brain ultrasound results showed that the incidence of brain injury was significantly higher in the study group than in the control group,the difference was statistically significant(P<0.05).Conclusions Craniocerebral ultrasonography is the first choice and dynamic monitoring method for neonatal craniocerebral examination.Neonatal craniocerebral ultrasound can be used to evaluate the effect of maternal gestational diabetes on neonatal brain,and can provide a basis for early diagnosis and intervention of neonates with gestational diabetes.
