多发性骨髓瘤从痰瘀论治

多发性骨髓瘤(Multiple myeloma,MM)是血液科的恶性肿瘤之一,目前仍被公认为是不可治愈的疾病。中医理论认为,痰饮、瘀血是恶性肿瘤形成的重要致病因素,但目前关于MM病因病机、病理、辨证论治等方面的认识并不多,多是各家之言,难以达成共识。本病若单治瘀血则痰浊不清,单治痰浊则瘀血不除。所以,笔者认为在MM的治疗上应以兼顾两者,本着痰瘀同治理论为治疗的根本大法,可以根据病因病机之辨证论治的不同,以化痰为主,或以祛瘀为主,但痰瘀胶着,密不可分。本文从痰瘀理论出发,解释MM的发病特点,以期待为防治该病开辟新的思路和方法,并在提高患者的生存质量、缓解疼痛、降低化疗副作用等方面得到明显的改善。

Treatment of multiple myeloma from phlegm and blood stasis

Multiple myeloma(MM) is one of the malignant tumors in hematology,and is still recognized as an incurable disease.The theory of traditional Chinese medicine holds that phlegm and retained fluid as well as blood stasis are the important pathogenic factors for the formation of malignant tumors.However,the current understanding of the etiology,pathogenesis,pathology,syndrome differentiation and treatment of MM is not rich,most of them hold different opinions making it difficult to reach a consensus.For this disease,in case of treatment of blood stasis alone,phlegm turbidity will not be eliminated; In case of treatment of phlegm turbidity alone,blood stasis will not be removed.Therefore,the author believes that the treatment of MM should give consideration to both,in line with the theory of treatment of both phlegm and blood stasis serving as the fundamental method of treatment,and according to the different syndrome differentiation and treatment of etiology and pathogenesis,the major therapeutic method should be to reduce sputum or remove blood stasis.However,phlegm and blood stasis are intermin-gled and inseparable.Based on the theory of phlegm and blood stasis,this paper explains the pathogenesis characteristics of MM,hoping to open up new ideas and methods for the prevention and treatment of such disease,and improve the quality of life of patients,relieve pain,reduce the side effects of chemotherapy.
