青少年外周动脉血栓的发病特点及超声影像特征

目的总结青少年外周动脉血栓患者临床发病特点及超声影像学特征,为提高诊治水平、降低致残率、改善预后提供临床依据。方法连续收集本院血管超声发现并经临床确诊的17例青少年外周动脉血栓患者资料,分析总结其临床发病特点及超声影像学特征。结果男性青少年好发(76.5%),17例青少年外周动脉血栓均发生于四肢动脉,其中1例伴发颈动脉血栓。伴发疾病中以自身免疫性疾病及心源性疾病最多见(各占17.6%)。多数发生于单侧肢体(88.2%)、下肢动脉好发(88.2%)、部位主要位于肢体中远段(76.5%)、血栓形态以新鲜血栓(88.2%)和急性不完全充填型为主(64.7%)。结论青少年外周动脉血栓主要好发于单侧下肢动脉中远段血管,主要诱因为心源性疾病和免疫性疾病所致;且以急性不完全充填型血栓为主。超声医师应提高对此类疾病的认识,提高诊治率,改善预后。

Ultrasonographic Features of Peripheral Arterial Thrombosis in Adolescents

Objective To summarize the clinical and ultrasonographic features of adolescent with extremities arterial thrombosis,provide clinical basis for the prevention and improvement of prognosis.Methods 17adolescents with arterial thrombosis in extremities detected by vascular ultrasound in Xijing hospital and confirmed by clinical symptoms were continuously collected,with clinical and ultrasonic data summarized.Results Extremities thrombosis of adolescents is common in male(counted for 76.5%).All 17cases were limbs thrombus(1case accompanied with carotid artery thrombus).Autoimmune disease and cardiogenic disease were the most common accompanied diseases(17.6%respectively).Extremities thrombosis showed higher incidence in unilateral limb(88.2%),especially in lower limbs(88.2%),and more easily shown at distal limb part(76.5%).Thrombus type were mainly acute fresh thrombus(88.2%)and incompletely occlusive thrombus(64.7%).Conclusions Peripheral arterial thrombosis in adolescents mainly occurs in the distal vascular of the unilateral lower extremity arteries,mainly caused by cardiogenic diseases and immune diseases;and acute incompletely filled thrombosis are common.Both clinical and ultrasound doctors should raise awareness of such diseases to improve diagnosis and prognosis.
