超声引导肾穿刺活检术后出现血肿的危险因素分析

目的探讨分析超声引导下经皮肾穿刺活检术后影响肾周及肾包膜下血肿产生的危险因素,以降低术后血肿的发生率。方法选取行超声引导下经皮肾穿刺活检患者155例,收集资料如患者性别、年龄、CKD分期、肾脏深度、穿刺用时、穿刺针数、操作医师、能否配合操作等,采用单因素及多因素分析相关因素对术后血肿产生的影响。结果单因素分析显示有无血肿组两组间CKD分期、肾脏深度、能否配合操作差异有统计学意义(P=0.03、0.01、<0.001)。多因素分析发现CKD分期>1期、肾脏深度≤20mm、患者不能配合操作是术后出现血肿的危险因素(P=0.009、0.010、<0.001)。结论超声引导下经皮肾穿刺活检术是一种诊断肾脏病变有效、安全的方法,多种因素会影响术后血肿的产生,训练患者呼吸配合操作可减少术后血肿的产生。

Analysis of the Risk Factors of Hematoma After Renal Biopsy Under Ultrasound Guidance

Objective To analyze different factors influencing the hematoma after renal biopsy under ultrasound guidance.Methods Multiple variables were taken a statistical analysis.Results There was a statistically difference between the two groups in CKD stage,depth of the renal and patients can cooperate the operation or not(P<0.05).The CKD stage>1,the depth of kidney≤20 mm and the patient’s inability to cooperate the operation were risk factors for postoperative hematoma(P<0.05).Conclusions Renal biopsy guided by ultrasound is an effective and safe method for renal disease and different factors can affect the occurrence of hematoma.
