腔内三维能量多普勒超声评估宫腔镜下子宫内膜搔刮术后的内膜容受性

目的探讨腔内三维能量多普勒(3D-PDA)超声评估体外受精-胚胎移植(IVF-ET)患者宫腔镜下子宫内膜搔刮术后子宫内膜容受性的应用价值,分析各超声参数的可靠性及灵敏性。方法选取我院行IVF-ET行宫腔镜下子宫内膜搔刮术患者102例,采用3D-PDA分别检测上述患者于手术前和手术后的两个移植周期前一日子宫内膜厚度、内膜分型、宫腔容积、内膜血流分型、内膜血流参数、3D-PDA参数和子宫动脉平均血流参数,比较两组间的上述指标是否存在差异;根据妊娠结局分为临床妊娠组与非妊娠组,比较上述指标在两组之间的差异。结果 (1)宫腔镜下子宫内膜搔刮术前和术后两组间子宫内膜分型和内膜血流分型比较有显著差异性(P<0.05);术后组3D-PDA参数(VI、FI、VFI)均高于术前组,两组间比较有统计学意义(P<0.05);患者宫腔镜术前与术后的子宫内膜血流参数(RI、PI)比较显示术前组高于术后组,具有统计学意义(P=0.002、0.001)。(2)临床妊娠组与非妊娠组间内膜血流参数(RI、PI)和3D-PDA血流参数(VI、FI、VFI)间比较有统计学意义(P<0.05),超声参数VI...

The Value Of Transvaginal Three-dimensional Power Doppler Ultrasound In Evaluating Endometrial Receptivity of Hysteroscopic Endometrial Minimally Invasive Surgery

Objective To investigate the value of transvaginal three-dimensional power doppler ultrasound in evaluating endometrial receptivity of the patients by hysteroscopic endometrial minimally invasive surgery,and then explore the reliability and sensitivity of ultrasound indexes.Methods 102 patients by hysteroscopic endometrial minimally invasive surgery were selected in the study,measuring ultrasonic parameters by using transvaginal three-dimensional power doppler ultrasound,comparing the difference of above indexes in the two groups.Results(1)The endometrial type and the endometrial blood flow type were statistically significant between the two groups of hysteroscopic endometrial minimally invasive surgery,as well as the blood flow parameters of endometrial(RI,PI)and the VI,FI,VFI(P<0.05).(2)The area under the ROC curve of ultrasonic parameters(VI,FI,VFI,RI,PI)was:0.929,0.791,0.704,0.821,0.814.Conclusions The transvaginal three-dimensional power Doppler ultrasound can monitore the ultrasonic parameters,the blood flow parameters of endometrial(RI,PI)and the 3 D-PDA parameters of endometrial(VI,FI,VFI)can reflect the endometrial perfusion,can be used as the ultrasonic indicators of endometrial receptivity assessment.The endometrial blood flow VI is one of the sensitive indicators for evaluating endometrial receptivity.
