围手术期抗凝策略对跟骨骨折术后伤口软组织愈合质量的影响

目的探讨围手术期抗凝策略对跟骨骨折术后伤口软组织愈合质量的改善效果。方法选取2016年4月～2017年8月本院收治的跟骨骨折患者70例作为研究对象,根据是否在围手术期给予抗凝治疗分为A、B两组,各35例。A组围手术期未给予抗凝策略治疗,B组围手术期给予抗凝策略治疗。对比两组患者受伤至手术时间、围手术期总引流量、切口干燥时间、住院时间、术后足跟功能恢复情况和手术切口愈合情况。结果 B组围手术期总引流量多于A组,且切口干燥时间长于A组(P0.05);在围手术期,A组住院时间短于B组(P0.05);A组AOFAS后足评分为(84.65±3.48)分,B组评分为(82.87±3.59)分,差异无统计学意义(P0.05);A组甲级愈合情况好于B组(P0.05)。结论抗凝策略治疗会增加经外科手术的跟骨骨折患者围手术期引流量,使其住院时间增长,并影响术后切口愈合质量。

Effect of perioperative anticoagulation strategy on the quality of wound soft tissue healing after calcaneal fractures

Objective To investigate the improvement effect of perioperative anticoagulation strategy on the healing quality of wound soft tissue after calcaneal fracture surgery.Methods From April 2016 to August 2017,70 patients with calcaneal fractures who were admitted to our hospital were selected as the study subjects.According to whether or not the anticoagulant therapy was given during the perioperative period,the patients were divided into group A and group B,with 35 cases in each group.Group A was not given anticoagulant strategy during perioperative period.Group B was given anticoagulation strategy during perioperative period.The time from injury to surgery,perioperative total amount of drainage,incision drying time,length of hospital stay,postoperative heel function recovery and surgical incision healing were compared between the two groups.Results The perioperative total amount of drainage in group B was more than that in group A,and the incision drying time was longer than that in group A(P<0.05); During the perioperative period,the length of hospital stay in group A was shorter than that in group B(P<0.05);The AOFAS posterior foot score in group A was (84.65±3.48),and the score in group B was (82.87±3.59).The difference was not statistically significant(P>0.05); Grade A healing in group A was better than that in group B (P<0.05).Conclusion Anticoagulation strategy can increase the perioperative amount of drainage of patients with surgical calcaneal fractures,increase the length of hospital stay,and affect the quality of postoperative incision healing.
