肥胖与多囊卵巢综合征患者临床特征的相关性分析

目的分层对照分析多囊卵巢综合征(PCOS)患者的临床特征,探讨肥胖对PCOS的临床意义及与其他临床特征的相关性。方法采用病例对照研究设计,选取Rotterdam标准诊断的PCOS患者301例为病例组,选取月经规律且有正常生育史的女性144例为对照组。采用问卷调查方法收集调查对象基本信息,根据体质指数(BMI)>25分为肥胖组和非肥胖组进行分层对照分析。分析PCOS与对照组各临床特征的差异以及肥胖与各种临床特征间的相关性。结果在445例观察对象中,肥胖组(84例)和非肥胖组(361例)的年龄、月经周期及HOMA-IR均有显著差异(均P<0.05);多囊卵巢和高雄激素血症的发生均无显著差异(均P>0.05);但高雄激素体征的发生肥胖组显著高于非肥胖组(P<0.05)。对肥胖组中的对照组和病例组相比较,显示两组在月经周期和HOMA-IR均有显著差异(均P<0.05)。将肥胖与PCOS各临床症状进行相关性分析,显示肥胖与月经周期、IR及高雄激素体征具有明显的相关性(P<0.05)。结论肥胖与IR相互作用影响PCOS女性代谢和生殖异常。

Analysis on the correlation between clinical features of PCOS and obesity

Objective Stratified comparing and analyzing the clinical features of patients with PCOS,discussing the clinical significance of obesity on PCOS and the correlation with other feature.Methods Case-control study of 301 cases of PCOS diagnosed by the Rotterdam standards compared with the 144 cases with normal menstruation and reproductive history.Collecting basic information of all the respondents through questionnaires,stratified comparing and analyzing the obesity group and non-obesity group divided by body mass index(BMI)(whether BMI>25 or not),and analyzing the differences of clinical feature and the cor-relation with obesity.Results Remarkable differences were found between the obesity group and non-obesity group on PCOS in age,menstrual cycle and HOMA-IR(all P<0.05).There is no significant difference between the two groups in hyperandrogenism and PCO(all P>0.05).But the incidence of hyperandrogensigns in obese group was significantly higher than non-obese group(P<0.05).In obesity patients,the result showed that remarkable differences were found in menstrual cycle,and HOMA-IRbetween the control and PCOS group(all P<0.05).To analysis the correlation between obesity and other symptoms of PCOS,results showed obesity with menstrual cycle,IR and hyperandrogen signs has significant correlation(P<0.05).Conclusion In patients with PCOS,abnormal metabolic and reproductive was caused or aggravated by obesity and IR.
