胎儿先天性肺囊腺瘤畸形与肺隔离症的产前超声诊断与预后分析

目的探讨胎儿先天性肺囊腺瘤畸形和肺隔离症病变的声像图特点与其预后。方法对于我院就诊的孕妇进行产前超声检查,诊断23例胎儿存在先天性肺囊腺瘤畸形和肺隔离症病灶,对其声像图进行回顾性分析,并对其追踪随访。结果超声诊断为先天性肺囊腺瘤畸形和肺隔离症的23例,病灶最大径15~72mm,平均(30.3±13.6)mm,胎儿病灶体积与头围比值为0.02~1.54,致胎儿心脏受压位移的病灶8例;均未合并其他器官异常,无全身水肿、胸腔或腹腔积液、羊水过多等征象。继续妊娠的19例胎儿均为足月活产,2例终止妊娠,2例失访。结论胎儿先天性肺囊腺瘤畸形和肺隔离症预后良好,产前超声探查病灶血供来源是鉴别二者的重要依据。

Prenatal Ultrasonic Diagnosis and Outcome Analysis of Congenital Cystic Adenomatoid Malformation of Lung and Bronchopulmonary Sequestration

Objective To investigate the ultrasonographic features and prognosis of congenital cystic adenomatoid malformation of lung(CCAM)and bronchopulmonary sequestration(PBS)in fetuses.Methods Pregnant women were examined with prenatal ultrasonography in our hospital.CCAM and PBS were diagwosed in 23 fetases.Prenatal ultrasonographic features of these lesions were retrospective analyzed and followed up.Results 23 cases with CCAM and BPS were diagnosed by prenatal ultrasound.The maximum diameter of these lesions were 15-72 mm,average(30.3±13.6)mm,and the cystic adenomatoid malformation volume ratio(CVR)of lesions were 0.02-1.54.Eight fetal hearts were pressured or moved by the lesions in those 23 cases.AU fetuses did not have other organs abnormalities and systemic edema,hydrothorax,ascites,hydramnios and other signs.Two pregnant women choose to terminate pregnancy.Two pregnant women were lost to follow-up.Another 19 pregnant women continued their pregnancies,pregnancy outcomes of their 19 fetuses were live births.Conclusions Fetuses with CCAM and PBS have a good prognosis.In prenatal ultrasound diagnosis,the source of blood supply of CCAM and BPS is an important evidence to distinguish them.
