低位直肠癌保肛手术治疗直肠癌的临床疗效与生活质量的改善效果观察

目的 对比低位直肠癌保肛术和腹会阴联合直肠癌根治术(Miles手术)治疗直肠癌的疗效和生活质量,为直肠癌的手术方案的选择提供有价值的参考依据。方法 选择2017年10月～2018年10月期间本院收治的65例直肠癌患者作为研究对象,随机分为A组和B组,A组患者采取低位直肠癌保肛术进行治疗,B组患者采取Miles手术进行治疗,对比两组患者手术治疗过程中的各项指标、不良反应发生率、复发率及生活质量。结果A组患者的术中出血量、术后排气时间、日排便次数以及住院时间明显小于B组,差异均有统计学意义(P0.05);A组患者不良反应发生率为11.4%,明显小于B组的30.0%,差异有统计学意义(P0.05),但两组患者一年内复发率差异无统计学意义(P0.05);两组患者术后生活质量比较差异均有统计学意义(P0.05),且治疗前后比较差异均有统计学意义(P0.05)。结论 与Miles手术相比,低位直肠癌保肛手术疗效更好,安全性更高,且患者的生活质量更好,值得临床推广。

Observation on the clinical efficacy and improvement of quality of life of rectal cancer treated with anus-sparing surgery for low rectal cancer

Objective To compare the efficacy and quality of life of rectal cancer with anus-preserving surgery and abdominal perineal resection Miles surgery on rectal cancer,and provide a valuable reference for the selection of rectal cancer surgical options.Methods A total of 65 patients with rectal cancer treated in our hospital from October 2017 to October 2018 were selected as the research subjects and randomly divided into group A and group B.The group A was treated with low rectal cancer anus-preserving surgery,and group B was treated with Miles surgery.The indexes,adverse reaction rate,recurrence rate,and quality of life were compared between the two groups of patients during surgical treatment.Results The intraoperative blood loss,postoperative exhaustion time,daily defecation times,and length of hospital stay were significantly lower in group A than those in group B,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in group A was 11.4%,significantly less than 30.0% of group B,and the difference was statistically significant(P<0.05).But the difference in recurrence rate within one year of the two groups of patients was not statistically significant(P>0.05).The postoperative quality of life comparison of the two groups was statistically significant(P<0.05),and the differences before and after treatment were statistically significant(P<0.05).Conclusion Compared with Miles surgery,anus-sparing surgery for lower rectal cancer has better efficacy,higher safety and better quality of life for patients,which is worthy of clinical promotion.
