儿童急性胃肠道移植物抗宿主病CT表现及文献复习

目的 观察儿童造血干细胞移植（HSCT）后急性胃肠道移植物抗宿主病（GI-GVHD）CT表现。方法 回顾性分析9例经临床或病理确诊的GI-GVHD患儿腹部CT特征，结合文献评价其诊断意义。结果 GI-GVHD患儿肠道CT表现为肠壁增厚、肠管扩张、肠腔积液、肠系膜水肿及肠系膜血管充盈、肠黏膜强化等，肠外表现包括汇管区水肿、胆囊壁增厚、胰腺增大、脾脏密度不均、膀胱壁增厚及腹腔积液等。结论 儿童HSCT后GI-GVHD腹部CT表现有助于临床早期诊断及评估疗效。

CT findings and literature review of children acute gastrointestinal graft-versus-host disease

Objective To observe the CT findings of acute gastrointestinal graft-versus-host disease (GI-GVHD) in children after hematopoietic stem cell transplantation (HSCT). Methods The abdominal CT features of 9 children with GI-GVHD confirmed clinically or pathologically were retrospectively analyzed, and the diagnostic significances of these features were evaluated combining with literature review. Results Intestinal CT manifestations of GI-GVHD included thicken intestinal, intestinal dilatation, effusion of intestinal cavity, mesenteric edema, mesenteric vascular filling and intestinal mucosa enhancement, etc. Extraintestinal CT findings included edema in the portal area, thicken gallbladder wall, enlargement of pancreas, uneven density of spleen, thicken bladder wall and peritoneal effusion, etc. Conclusion Abdominal CT findings of GI-GVHD children after HSCT had some characteristics were helpful to early clinical diagnosis of GI-GVHD and evaluation of curative effect.
