以自控无创电刺激穴位治疗为主的两阶段序列疗法治疗单侧难治性原发性耳鸣的探究＊

目的 探讨以自控无创电刺激穴位治疗为主的两阶段序列疗法对单侧难治性原发性耳鸣的治疗效果。方法 选择单侧难治性原发性耳鸣患者42例,利用研究者发明的基于智能手机和蓝牙通信控制的耳鸣经络穴位治疗仪分别对听宫穴-翳风穴,听宫穴-中渚穴以及翳风穴-中渚穴三组穴位依次进行自控电刺激治疗,2周一个疗程,共治疗2个疗程。第一疗程后,加用激素治疗1周,同时进行改善微循环治疗2周。采用耳鸣严重程度评估量表于治疗第2周、第4周、治疗结束后1个月及3个月进行疗效评估。结果 治疗2周后,4例痊愈,3例显效,17例有效,18例无效,总有效率57.14%。治疗4周后6例痊愈,8例显效,16例有效,12例无效,总有效率71.43%。所有治疗过程中无听力减退、失眠、局部损伤等副作用。治疗结束后1月,6例痊愈,8例显效,16例有效,12例无效,总有效率71.43%;治疗结束后3月,4例痊愈,7例显效,15例有效,16例无效,总有效率61.90%;治疗结束1月和3月的随访过程中,均无患者反映治疗过程中及治疗结束后有听力减退、局部损伤等任何副作用。结论 以自控无创电刺激穴位治疗为主的两阶段序列疗法是一种安全、便捷、有效的耳鸣治疗方法,可以惠及耳鸣患者,具有广阔的应用前景。

Preliminary Observation on Curative Effect of Two-stage Sequential Therapy with Self-controlled Non-invasive Acupoint Electrical Stimulation Therapy as the Main Treatment in Unilateral Refractory Primary Tinnitus

Objective To investigate the therapeutic effect of two-stage sequential therapy with self-controlled non-invasive electrical stimulation acupoint therapy on unilateral refractory primary tinnitus.Methods A cohort of 42 patients with unilateral refractory primary tinnitus were enrolled for the combined therapies. In detail, three pairs of acupoints, including Tinggong (SI19)-Yifeng (TE17), Tinggong (SI19)-Zhongzhu (TE3) and Zhongzhu (TE3)-Yifeng (TE17), were selected and stimulated in sequence for 15 minutes twice a day. The whole therapy took two weeks for one course and had two sequential courses. After the first course of acupoint electrical stimulation, glucocorticoid and other microcirculation improving medicines were added during the second course. The stimulation was based on smartphone and Bluetooth technology, making the therapeutic process effective, non-invasive and self-controlled. All the post-treatment patients in this cohort were followed up at two weeks, four weeks, one month, and three months for tinnitus severity assessment.Results In this cohort, the two weeks'' total effective rate was 57.14% and the details were as follows: 4 cured cases, 3 markedly effective cases, 17 effective cases, and 18 ineffective cases. And the four weeks'' total effective rate was 71.43% and the details were as follows: 6 cured cases, 8 markedly effective cases, 16 effective cases, and 12 ineffective cases. No side effects were found during the treatments. Furthermore, one month after the whole treatment, there were 6 cured cases, 8 markedly effective cases, 16 effective cases, and 12 ineffective cases with a total effective rate of 71.43%. At last, three months after the whole treatment, there were 4 cured cases, 7 markedly effective cases, 15 effective cases, and 16 ineffective cases with a total effective rate of 61.90%. During the follow-up period of one month and three months after the treatment, no patients reported any side effects, such as hearing loss and local injury during and after the treatment.Conclusion The acupoints electrical stimulation was non-invasive and self-controlled. The two-stage sequential therapy with acupoint electrical stimulation as the main method is a safe, convenient and effective tinnitus treatment method, which has a good potential to benefit tinnitus patients and be broadly put into use.
