Chiari畸形并脊髓空洞症的显微外科治疗

目的探讨显微外科技术对Chiari畸形并脊髓空洞症的治疗效果。方法127例Chiari畸形并脊髓空洞症经MRI确诊后应用显微外科技术,采用颅后窝减压术,并进行平均1.5年以上随访以判断治疗效果。结果颅后窝减压术后随访89例,术后临床症状均有不同程度的缓解,并存疾病如脊髓空洞症明显改善。结论对Chiari畸形并脊髓空洞症宜采用显微外科手术治疗,可根据病情特点选择小范围颅后窝减压、恢复脑脊液循环、枕大池扩大修补术等外科术式。

Microsurgical treatment of Chiari's malformation associated with syringomyelia

Objective To probe the treatment effects of the Chiari's malformation associated with syringomyelia using microsurgical technique.Methods The127patients with Chiari's malformation associated with syringomyelia confirmed by CT or MRI were analyzed retrospectively.The patients underwent posterior cranial fossa decompression with microsurgery.All the patients were followed-up for more than1.5years and the treatment results were evaluated.Results Improvement of associated syringomyelia and relief of the clinical symptoms were achieved in89patients who underwent posterior cranial fossa decompression.Conclusion The patients of Chiari's malformation associated with syringomyelia should be microsurgically treated.The posterior cranial fossa decompression on a small scale,recovering cerebro-spinal circulation and extending and repairing foramen magnum pool may be a method of surgical treatment for syringomyelia according to the condition of disease.
