急性脑梗死患者血清正五聚蛋白3水平与其血脂、认知功能及预后的关系研究

摘    要:目的探索急性脑梗死(ACI)患者血清正五聚蛋白3(PTX3)水平与血脂、认知功能及预后的关系。 方法回顾性分析2018年4月至2020年10月期间海南西部中心医院收治的94例急性ACI患者的病例资料。选择同期92名健康体检者作为对照组。对两组受试者进行血脂、血清PTX3水平检测,评估改良长谷川痴呆量表(HDS-R)评分、Rankin(mRS)评分并进行组间比较。同时再根据观察组颈动脉超声检查结果分为两组,即不稳定组(n=35,不稳定斑块)、稳定组(n=59,稳定斑块),比较两组mRS评分、HDS-R评分及血脂、PTX3水平后,采用Pearson相关分析及多元逐步回归分析PTX3水平与HDS评分、mRS评分及血脂水平相关性。 结果观察组PTX3、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)均高于对照组,高密度脂蛋白胆固醇(HDL-C)、HDS和mRS评分均低于对照组,差异均有统计学意义(均P<0.05)。同时不稳定组患者的PTX3、TC、TG、LDL-C、mRS评分均高于稳定组,HDL-C、HDS评分低于稳定组,差异均有统计学意义(均P<0.05)。经Pearson法分析,PTX3水平与TC、TG、LDL-C、mRS评分呈正相关性(r=0.808、0.506、0.833、0.769,均P<0.001),与HDS评分呈负相关性(r=-0.419,P<0.001)。同时经多元逐步回归分析,TC、LDL-C、mRS评分会对PTX3产生显著的正向影响关系(t=2.673、3.579、2.708,P<0.05)。 结论急性脑梗死患者PTX3水平表达明显升高,且与HDS评分呈负相关性,对TC、TG、预后可产生正向影响。

Abstract:ObjectiveTo explore the relationship between serum n-pentamerin 3 (PTX3) level and blood lipids, cognitive function, and prognosis in patients with acute cerebral infarction (ACI). MethodsThe clinical data of 94 patients with ACI (observation group) treated at Hainan Western Central Hospital from April 2018 to October 2020 were analyzed retrospectively. Ninety-two healthy people receiving health check in the same period were selected as controls. Blood lipids and serum PTX3 levels of the two groups were detected, and the scores of modified Hasegawa Dementia Scale (HDS-R) and modified Rankin Scale (mRS) were evaluated and compared between the two groups. The observation group was divided into two groups according to the results of carotid ultrasound examination, namely, unstable group (n=35, with unstable plaque) and stable group (n=59, with stable plaque). The mRS score, HDS-R score, blood lipids, and PTX3 level in the two groups were compared. Pearson correlation analysis and multivariate stepwise regression analysis were performed to explore the correlation between PTX3 level and HDS-R score, MRS score, and blood lipid levels. ResultsPTX3, total cholesterol (TC), triglyceride (TG), and low density lipoprotein cholesterol (LDL-C) in the observation group were all higher than those in the control group, and high density lipoprotein cholesterol (HDL-C), HDS-R score, and mRS score in the observation group were all lower than those in the control group (P<0.05). PTX3, TC, TG, LDL-C, and mRS score in the unstable group were all higher than those in the stable group, and HDL-C and HDS-R score were both lower than those in the stable group (P<0.05). Pearson correlation analysis showed that the level of PTX3 was positively correlated with TC, TG, LDL-C, and mRS (r=0.808, 0.506, 0.833, and 0.769, respectively, P<0.001), and negatively correlated with the score of HDS-R (r=-0.419, P<0.001). Multiple stepwise regression analysis showed that TC, LDL-C, and mRS had a significant positive impact on PTX3 (t=2.673, 3.579, and 2.708, respectively, P<0.05). ConclusionThe expression of PTX3 in patients with ACI is significantly increased, and it is negatively correlated with HDS-R score, and has a positive effect on TC, TG, and prognosis.
