2017—2020年青海省人民医院综合ICU医院感染目标监测分析

摘    要:目的分析青海省人民医院综合重症监护室(ICU)医院感染目标性监测结果,为高原地区ICU医院感染目标预防策略提供依据。 方法对2017—2020年青海省人民医院综合ICU目标监测结果和器械污染所致相关医院感染情况进行回顾性分析。 结果共监测综合ICU患者6283例,医院感染发病率为6.20%,医院感染例次发病率为6.94%;感染类型以呼吸系统感染为主,占58.68%,其次为泌尿系统感染,占17.58%;呼吸机相关性肺炎(VAP)发病率为8.88‰,导尿管相关泌尿系感染(CAUTI)发病率为2.53‰,血管导管相关血流感染(CLABSI)发病率为0。日感染发病率2017年为15.09‰,2020年为8.38‰。 结论青海省人民医院综合ICU日感染发病率降低,持续开展ICU医院感染目标性监测,及时发现危险因素并采取干预措施,可有效降低ICU医院感染发病率。

Abstract:ObjectiveTo analyze the status of targeted surveillance of nosocomial infection in intensive care unit (ICU) of Qinghai Provincial People′s Hospital to provide a basis for nosocomial infection prevention strategy in hospitals at high altitude. MethodsBy means of retrospective survey, the data of targeted surveillance of nosocomial infection and device-related infection in ICU of Qinghai Provincial People′s Hospital from January 2017 to December 2020 were collected. ResultsTotally 6283 patients were enrolled in the study. The incidence of nosocomial infection was 6.20%, and the case-time infection rate was 6.94%. The patients with respiratory system infection were dominant among the patients with nosocomial infection, accounting for 58.68%, followed by urinary system infection, accounting for 17.58%. Among the patients with device-related infection, the incidence of ventilator-related pneumonia (VAP) was 8.88‰, the incidence of catheter associated urinary tract infection (CAUTI) was 2.53‰, and the incidence of catheter-related bloodstream infection (CRBSI) was 0. The daily VAP rate was 15.09‰ in 2017 and 8.38‰ in 2020. ConclusionThe incidence of ICU device-associated infection shows an overall downward trend. The incidence of nosocomial infection in ICU can be effectively reduced by carrying out the targeted monitoring of nosocomial infection in ICU, discovering the risk factors in time, and taking intervention measures.
