超声清创水刀联合封闭负压引流技术在糖尿病足溃疡中的应用

摘    要:目的探讨超声清创水刀联合封闭负压引流技术在治疗慢性糖尿病足溃疡中的临床效果。方法选取2018-2020年诊疗的40例糖尿病足溃疡病人,随机分为观察组和对照组,各20例。观察组采用超声清创水刀联合封闭负压引流技术治疗,对照组采用传统外科清创联合封闭负压引流技术。对比2组病人的溃疡治疗效果、疼痛度、踝肱指数及并发症发生率。结果第1次清创术后观察组创面表面情况、肉芽组织生长及炎性表现方面均优于对照组(P < 0.05)。治疗后2组疼痛程度和ABI均较治疗前减轻(P < 0.05~ P < 0.01),且治疗后观察组疼痛程度和ABI改善均优于对照组,差异有统计学意义(P < 0.05)。2组治疗后感染、植皮或皮瓣术后成活情况比较差异有统计学意义(P < 0.05)。结论在糖尿病足溃疡治疗过程中应用超声清创水刀联合负压引流技术中,可以增加溃疡创面愈合能力,减少并发症的发生,改善病人的生存质量。

Application of the ultrasonic waterjet combined with vacuum sealing drainage in diabetic foot ulcer

Abstract:ObjectiveTo investigate the clinical effects of the ultrasonic waterjet combined with vacuum sealing drainage(VSD) in the treatment of chronic diabetic foot ulcer.MethodsA total of 40 diabetic foot ulcer patients from January 2018 to December 2020 were selected, and randomly divided into observation group and control group(20 cases in each group).The observation group was treated with ultrasonic waterjet combined with VSD, and the control group was treated with traditional surgical debridement combined with VSD.The treatment effects, pain degree, ankle-brachial index and complication rate between two groups were compared.ResultsAfter the first debridement, the wound surface, granulation tissue growth and inflammatory manifestations in observation group were better than those in control group(P < 0.05).After treatment, the pain degree and ABI in two groups were reduced compared with those before treatment(P < 0.05 to P < 0.01), and the improvement of pain degree and ABI in observation group were better than those in control group after treatment(P < 0.05).The differences of the infection and survival of skin grafting or flap between two groups after treatment were statistically significant(P < 0.05).ConclusionsThe application of ultrasonic waterjet combined with VSD in the treatment of diabetic foot ulcer can increase the healing ability of ulcer wound, reduce the occurrence of complications, and improve the quality of life of patients.
