2017—2021年某市医疗机构放射工作人员职业性外照射个人剂量监测结果分析

摘    要:目的 分析某市医疗机构放射工作人员职业性外照射个人剂量水平,为放射防护管理提供参考依据。方法 依据《职业性外照射个人监测规范》,采用热释光剂量测读系统对放射工作人员进行职业性外照射个人剂量监测,将每年监测4次及以上的人员作为研究对象,对监测结果进行汇总与分析。结果 2017—2021年共收集3 010人个人剂量监测结果,放射工作人员年有效剂量中位数为0.11 mSv·a-1。98.84% 放射工作人员的年有效剂量≤1.0 mSv;5人年有效剂量>5.0 mSv。2019年放射工作人员年有效剂量中位数为0.10 mSv·a-1,低于2017年的0.11 mSv·a-1、2018年的0.12 mSv·a-1和2020年的0.12 mSv·a-1(P均<0.05);核医学放射工作人员年有效剂量中位数为0.08 mSv·a-1,低于诊断放射学的0.11 mSv·a-1、牙科放射学的0.14 mSv·a-1、放射治疗的0.10 mSv·a-1和介入放射学的0.12 mSv·a-1,介入放射学高于诊断放射学(P均<0.05);三级医院放射工作人员年有效剂量中位数为0.11 mSv·a-1,高于一级医院(P<0.05)。结论 2017—2021年某市医疗机构放射工作人员职业性外照射个人剂量处于低水平,应重点关注三级医疗机构的放射防护管理以及核医学和介入放射学放射工作人员的职业健康管理。

Analysis of personal dose monitoring results of occupational external radiation for radiology staff in a municipal medical institution from 2017 to 2021

Abstract:Objective To analyze the dose level of occupational external radiation of radiation staff in a municipal medical institution, so as to provide a reference for radiation protection management. Methods According to the “Occupational External Radiation Personal Monitoring Specification”, the thermoluminescence dose reading system was used to monitor the individual measurement of occupational external radiation of radiation staff, and the monitoring results were summarized and analyzed by taking the personnel monitored 4 times a year or more as the research objects. Results A total of 3 010 individuals were collected from 2017 to 2021, and the median annual effective dose of radiologists was 0.11 mSv·a-1. The annual effective dose of 98.84% of radiologists ≤ 1.0 mSv. The annual effective dose for 5 people>5.0 mSv. The median annual effective dose for radiation workers in 2019 was 0.10 mSv·a-1, down from 0.11 mSv·a-1 in 2017 and 0.12 mSv·a-1 in 2018 and 0.12mSv·a-1 in 2020 (all P<0.05). The median annual effective dose for nuclear medicine radiology staff was 0.08 mSv·a-1 lower than 0.11 mSv·a-1 for diagnostic radiology, 0.14 mSv·a-1 for dental radiology, 0.10 mSv·a-1 for radiotherapy, and 0.12 mSv·a-1 for interventional radiology, interventional radiology was higher than 0.11 mSv·a-1 for diagnostic radiology (P<0.05); The median annual effective dose of radiology staff in tertiary hospitals was 0.11 mSv·a-1, which was higher than that of primary hospitals (P<0.05). Conclusion From 2017 to 2021, the personal dose of occupational external radiation for radiology staff in a municipal medical institution is at a low level, and the focus should be on the radiation protection management of tertiary medical institutions and the occupational health management of radiation staff in nuclear medicine and interventional radiology.
