渐进性肌肉放松训练对胫腓骨骨折术后患者疼痛及康复的影响

摘    要:目的 探讨渐进性肌肉放松训练对胫腓骨骨折术后患者疼痛及康复的影响。方法 选取2019年1月—2020年5月本院收治的胫腓骨骨折患者97例,按照随机数字表法分为对照组(47例)和研究组(50例)。对照组采用常规康复护理,在对照组基础上,研究组采用渐进性肌肉放松训练,比较两组术后康复指标、疼痛程度、膝关节功能、并发症。结果 护理后,研究组术后首次下床时间(11.65±2.38) d、住院时间(13.24±3.12) d、骨折愈合时间(14.36±2.22) d均较对照组(14.21±2.84、16.35±3.47、17.65±2.13)短(t=4.822、4.647、7.439,P<0.001)。出院前,两组NRS评分均降低,且研究组(1.95±0.35)分较对照组(2.67±0.33)分低(t=10.409,P<0.001),。随访3个月,两组Lysholm评分均提升,且研究组(87.32±6.14)分高于对照组的(78.65±5.84)分(t=7.116,P<0.001)。研究组并发症发生率为4.00%,较对照组的17.02%低(χ2=5.828,P<0.05)。结论 渐进性肌肉放松训练可减轻胫腓骨骨折术后患者疼痛程度,缩短骨折愈合时间,降低并发症发生率,促进膝关节功能恢复。

Abstract:Objective To explore the effect of progressive muscle relaxation training on pain and rehabilitation of patients after tibia and fibula fracture operation. Methods From January 2019 to May 2020, 97 patients with tibia and fibula fractures were selected and randomly divided into a control group (47 cases) and a trial group (50 cases). The control group received routine rehabilitation nursing, and on the basis of the control group, the trial group adopted progressive muscle relaxation training, and the postoperative rehabilitation index, pain degree, knee joint function and complications were investigated and compared between the two groups. Results After the nursing, the average first time out of bed , hospitalization time , and fracture healing time in the trial group were shorter than those in the control group (14.21±2.84, 16.35±3.47, 17.65±2.13)(t=4.822, 4.647, 7.439; all P<0.001). Before discharge, the numerical rating scale (NRS) scores of both groups decreased, and the average score of the trial group (1.95±0.35) was lower than that of the control group (2.67±0.33) (t=10.409, P<0.001). After 3 months of follow-up, the Lysholm scores of both groups were improved, and the average scores of the trial group (87.32±6.14) was higher than that of the control group (78.65±5.84) (t=7.116, P<0.001). The incidence of complications in the trial group was 4.00%, lower than that in the control group (17.02%), and there was a significant difference between the two groups (χ2=5.828, P<0.05). Conclusion The progressive muscle relaxation training can reduce the pain degree of patients with tibia and fibula fracture after operation, shorten the healing time of fracture, reduce the incidence of complications, and promote the functional recovery of knee joint.
