摘    要:目的:探讨气道内悬挂125I粒子2种不同剂量治疗中心型肺癌引起的气道狭窄的疗效和不良反应。方法:采用环甲膜穿刺、置管、气道内分别悬挂单列(A组)、双列(B组)125I放射性微粒子,对中心型肺癌气道狭窄进行持续近距离放疗,治疗3个月。结果:22例患者中,A组气道狭窄缓解率为63%,B组气道狭窄缓解率为81%,2组缓解率无明显差异(P>0.05);呼吸道症状和胸痛的缓解率也无明显差异(P>0.05),PEF%有明显差异(P<0.05)。结论:气道内悬挂125I粒子治疗癌性气道狭窄有较好疗效,种子源单列或双列排列对气道狭窄缓解率无明显差异,悬挂双列种子源对改善肺功能更有帮助。

Abstract:Objective:To evaluate the effectiveness and safety of two different irradiation dosages of endobronchial ~ 125I seeds for malignant airway stenosis induced by central lung cancer. Methods: Endobronchial brachytherapy was performed by implanting and suspending one (Group A) or two rows (Group B) of ~ 125I seeds in airway via cricothyroid membrane puncture. Radiotherapy lasted three months. Results:Of 22 cases,airway stenosis was ameliorated in 63% patients in group A and 81% patients in group B (complete response rate part response rate). There was no significant difference between the two groups(P> 0.05). The respiratory symptoms and chest pain were relieved in both groups with no significant difference(P>0.05). But the percentage of peak expiratory flow (PEF%) was significantly different between the two groups(P<0.05). Conclusion:Suspension of ~ 125I seeds in airway is effective in management of malignant airway stenosis. No significant difference was found between the two irradiation dosages of ~ 125I seeds. Implantation of two rows of ~ 125I seeds was more helpful to improve lung function.
