中青年视网膜静脉阻塞临床分析

目的:探讨中青年视网膜静脉阻塞(RVO)的临床特点。方法:回顾分析36例36眼40岁以内中青年RVO的临床表现、类型、诊断、治疗及预后。随访时间3~24mo,观察病情演变及预后。结果:36例中视网膜中央静脉阻塞(CRVO)22例,分支静脉阻塞(BRVO)14例,其中黄斑小分支静脉阻塞(MBR-VO)2例。经应用皮质类固醇等药物治疗后多数患者症状改善,视力提高。随访中有2例视网膜出现了新生血管及无灌注区,另有2例出现了黄斑水肿,共4例接受了激光治疗。结论:中青年RVO患病率低,发病可能与炎症有关,对药物治疗的反应相对明显,一般为良性病程,预后较好。但有少数病例可出现视网膜新生血管(5.6%)及黄斑囊样水肿,需适时激光治疗。

Clinical analysis of retinal vein occlusion in the young and middle-aged people

AIM: To explore the clinical characteristics of retinal vein occlusion (RVO) among young and middle-aged people.·METHODS: A retrospective analysis was carried out in 36 cases (36 eyes) with RVO aged less than 40 years old in terms of clinical manifestations, types, diagnosis, treatment and prognosis. The follow-up period lasted 3 to 24 months, and the disease evolution and prognosis were observed. ·RESULTS: Among 36 cases, there were 22 cases with central retinal vein occlusion (CRVO), 14 cases with branch retinal vein occlusion (BRVO) in which 2 cases had macular branch retinal vein occlusion (MBRVO). The conditions of most patients were improved and visual acuity was raised after medicines such as corticosteroids were applied. During the follow-up, retinal neovascula-rization and non-perfusion regions occurred in 2 cases and macular edema in another 2 cases. Therefore these 4 cases underwent laser therapy.·CONCLUSION: The incidence of RVO among young and middle-aged people is low,and disease onset may be associated with inflammation.Patients have comparative-ly obvious reactions to medication. Usually the course of disease is benign and the prognosis is good.However,retinal neovascularization(5.6%)and cystoid macular edema may occur in a few cases and timely laser therapy is needed.·
