精神分裂症患者血中S100b蛋白和抗脑抗体的研究

摘    要:目的探讨血浆S100b浓度及血清抗脑抗体(ABAb)与精神分裂症的关系。方法对符合国际疾病分类第10版(ICD-10)精神分裂症诊断标准的57例患者(首发34例,慢性23例),于治疗前、第12周末评定PANSS量表,同时采集血标本,并以60名正常人为对照,用ELISA测定血浆S100b及血清ABAb。结果①患者治疗前、后S100b浓度均明显高于对照组(P均小于0.001),且治疗前的浓度高于治疗后。②患者组治疗前S100b浓度与PANSS 总分和阴性症状(N)分量表分均呈正相关(P均小于0.05)。③患者组治疗前、后ABAb阳性率均显著高于对照组(P均小于0.005)。④ABAb阳性患者的S100b明显高于ABAb阴性患者(P<0.05)。结论分裂症患者血中S100b 浓度和ABAb阳性率均明显高于正常人,且S100b浓度与患者精神症状严重程度密切相关,提示S100b蛋白可能是观察精神分裂症患者病情变化的新指标。

Abstract:Objective To explore the relationship between anti-brain antibody, level of S100b protein and schizophrenia. Methods Fifty seven schizophrenic inpatients who met the diagnostic criteria of ICD-10 were enrolled in the study. Among them, 34 patients were first-episode, 23 patients were chronic padents. All patients were assessed with PANSS and taken blood samples at baseline and 12th weekend. The anti-brain antibody and level of S100b protein were analyzed with ELISA technique. Results (1) The levels of S100b protein before and after treatment of the schizophrenics respectively were significantly higher than those of normal controls (P1< 0.001; P2 < 0.001); the level of S100b protein in padents before treatment was significantly higher than that level after treatment. (2) The baseline level of S100b protein was positively correlated with total score of PANSS and score of negative subscale(P1 = 0.043; P2 = 0.021). (3) The positive rates of arid-brain antibody in schizophrenics before and after 12-week neuroleptic treatment respectively were significantly higher than those of normal controls ( P < 0.005). (4) The level of S100b protein in padents who had positive anti-brain antibody was significantly higher than that in padents with negative anti-brain antibody (P = 0.043). Conclusions The anti-brain antibody may be involved in the autoimmune system. The higher level of S100b was associated with severe psychopathology. The level of S100b protein in padents may be a new index that can indicate patients' condition.
