正常成人上气道鼾症相关检测指标的MRI研究

摘    要:目的建立正常成人上气道鼾症相关检测指标的正常参考值,为鼾症的定位和定性诊断提供依据。方法对38例无鼾症受检者上气道进行MRI检查,测量其软腭长、腭咽距、舌咽距、舌体厚、舌咽角、会咽角,并计算腭咽指数、舌咽指数。同时分别测量鼻咽、口咽和喉咽部各自扩张期和塌陷期的截面积。结果腭咽指数和舌咽指数的大小反映腭咽和舌咽腔的矢状径大小的变化。腭咽指数和舌咽指数增大则咽腔变窄。鼻咽部在呼吸时活动度不大,而口咽和喉咽部截面积则可随呼吸状态发生变化。结论上气道鼾症相关静态径线测量和动态截面积的变化结合起来,可更真实地反映上气道解剖形态和功能状态的变化。

MRI Study of Snoring-related Measurement Indexes in the Upper Airway of Normal Adult

Abstract:Objective To establish a normal reference value of snoring-related measurement indexes in normal adult upper airway for providing the basis for the located and etiological diagnosis of snoring. Methods The upper airway of 38 cases of non-snoring subjects were performed by MRI. The length of soft palate, distance of velopharyngeal, distance of glossopharyngeal, thickness of the body of tongue, glossopharyngeal angle and epiglottis-pharyngeal angle were measured, and the palatopharyngeal index and glossopharyngeal index were calculated. Simultaneously, the cross-sectional area of nasopharynx, oropharynx and hypopharynx were measured in their expansion phase and collapse phase respectively. Results The palatopharyngeal index and glossopharyngeal index reflected the change of palatopharyngeal cavity and glossopharyngeal cavity in the sagittal distance. The palatopharyngeal index and glossopharyngeal index were increased, then the pharyngeal space were narrowed. The nasopharynx hadn’t a visible mobility during respiration, while the cross-sectional area of oropharynx and hypopharynx could be changed with varying respiratory conditions. Conclusion It could be more truly reflect the changing of the anatomic morphology and functional status in upper airway when the static snoring-related distance measurement indexes and dynamic change of cross-sectional area were combined.
