胸水及血浆NT-proBNP对老年患者心源性胸腔积液的诊断价值

目的：研究老年心力衰竭（HF）急性加重期合并胸水患者的胸水与血浆NT—proBNP相关性和诊断价值。方法：选择2007年1月-2008年12月入住本院的老年胸腔积水患者60例，平均年龄（81．0±8．03）岁。取胸腔积水行生化、细菌学、细胞学检查NT—proBNP水平测定。分析老年患者胸腔积水和血浆NT—proBNP水平对诊断心源性胸腔积液的应用价值。结果：老年患者胸腔积水和血浆NT—proBNP水平无显著差异[（6537．1±10299．6）：（6085．6±10147．5）ng／L]（P〉0．05），两者呈线性正相关（R=0．955，P〈0．05）。在诊断心源性胸腔积液方面，胸腔积水NT—proBNP cutoff值为1305．0ng／L，血浆NT—proBNP cutoff值为1723．0ng／L。结论：老年患者胸水和血浆NT—proBNP水平可以作为诊断心源性胸腔积液的一项辅助性检查指标。

The diagnostic value of NT-proBNP in hydrothorax or plasma for the elderly who had heart sever failure

Objective： To study the diagnostic value of NT-proBNP in hydrothorax or plasma for the elderly who had heart sever failure. Method：60 patients older that 65 years old（81. 0±8.03 years）who were admitted in our hospital from January 2007 to December 2008 were selected. The biochemical, bacteriologic test,cytology and NT-proBNP level of hydrothorax or plasma were detected. Result.. There is no significant difference between the NT proBNP level in hydrothorax and plasma （ P 〉0.05）. There is a linear correlation between the NT-proBNP level of hydrothorax and plasma （R=0. 955, P 〈0.05）. When hydrothorax NT-proBNP≥1305.0 ng/L ,both the sensitivity and specificity are 83.33%, and when plasma NT-proBNP ≥ 1723.0 ng/L, the sensitivity is 83.33% and specificity is 91. 67%. Conclusion：The NT- proBNP level of hydrothorax and plasma was an effective and reliable diagnostic tool to diagnose the elderly cardiogenic pleural effusion.
