P-选择素和胰岛素抵抗对糖尿病肾病的影响

摘    要:目的 研究P-选择素、胰岛素抵抗(IR)与糖尿病肾病(DN)的关系。方法 对109例2型糖尿病(T2DM)患者(有肾病者为DN组、无肾病者为DM组)和正常对照组(NC),进行空腹血糖(FPG)、胰岛素、P-选择素测定,计算HOMA法胰岛素抵抗指数(HOMA-IR)。结果 DN和DM组血中的FPG、胰岛素、P-选择素及HOMA-IR均高于NC组(P〈0.01),且DN组上述结果都高于DM组(P〈0.01),相关分析表明IR与P-选择素呈正相关,多元回归显示DN发生与高FPG、高胰岛素、IR及P-选择素相关。结论P-选择素参与了IR,且P-选择素与IR一起参与了他T2DM患者DN的发生、发展过程。

Abstract:Objective To study the association between P-selectin,insulin resistance(IR) and diabetic nephropathy(DN). Methods Fresh plasma glucose(FPG)、insulin and P-seleetin were detected in 109 cases with type 2 diabetes mellitus(T2DM) ,which were seperated into groups : Group DN, with nephropathy ; group DM, without nephropathy and normal control, then calculate the value of HOMA-IR. Results FPG、insulin、P-selectin and HOMA-IR in group DN and DM were remarkably higher than that in normal controls( P 〈0.01 ) ,and the results above in Group DN were higher than that in Group DM ( P 〈0.01 ). Related analysis showed that IR had positive correlation with P-selectin, multiple linear regression displayed that DN genesis had association with high FPG, hyper insulin, IR and P-selectin. Conclusion P-selectin has participate IR and together with IR participate the course of DN's genesis and development in T2DM.
