经腹联合断流治疗门脉高压症的手术并发症300例

目的：探讨联合断流术（改良Sugiura术）治疗门静脉高压症引起的食管下端胃底曲张静脉破裂出血的安全性和有效性。方法：对我院1997年1月-2007年10月治疗的300例门脉高压症并出血的病例进行回顾性总结分析。结果：38例患者出现术后并发症，术后并发症发生率为12．7％，11例患者出现术后发热，腹腔大出血和胃肠道功能紊乱分别出现8例和7例，胃坏死和食道损伤各5例，门静脉血栓患者2例。结论：应用该术式后患者发生并发症少，死亡率低，远期疗效满意，是防治门静脉高压症引起的食管下端胃底曲张静脉破裂出血的理想术式。

Analysis of the complications of 300 cases with portal hypertension treated with the combined esophagogastric devascularization

Objective：To investigate the safety and efficacy of the combined esophagogastric devascularization in the treatment of esophagogastric variceal bleeding caused by portal hypertension. Method：The treatment of 300 patients who underwent the Combined esophagogastric devascularization between January 1997 and October 2007 were retrospectively analyzed. Result：38 cases were found post-operative complication . the total incidence of postoperative complication was 12.7% （38/300）. 11 cases were postoperative fever. Abdominal hemorrhage and func tional disturbances of gastrointestinal tract were 8 and 7 cases respectively. Esophageal injury and gastric necrosis all were 5 cases. Portal vein thrombosis was only 2 cases. 38 cases were found to have post operative complica- tions. The total incidence of post-operative complications was 12.7%. There were 11 cases with postoperative fe ver. 8 cases were found to have abdominal hemorrhage and 7 cases had functional disturbances of gastrointestinal tract. There were esophageal injury and gastric necrosis in all 5 cases. 2 cases were found to have portal vein thrombosis. Conclusion：The Combined esophagogastric devascularization is a ideal surgical procedure in the treatment of portal hypertension.
