急性输尿管结石872例

目的：比较钬激光碎石术（HLL）与体外冲击波碎石（ESWL）治疗急性输尿管结石的疗效和实用性。方法：采用HLL治疗急性输尿管结石109例，ESWL碎石763例。比较2组治疗时间、单次治愈率的差异。结果：ESWL碎石时间平均为16min，HLL时间平均为27min，两者比较差异有统计学意义（P〈0．05）。ESWL单次成功率82．4％（629／763），HLL单次成功率为90．8％（99／109），两者比较差异有统计学意义（P〈0．05）。结论：ESWL简单、有效，是简单性急性输尿管结石首选的治疗方法。HLL单次成功率较高，是复杂性急性输尿管结石首选的治疗方法。

Treatment of 872 cases for acute ureteral calculi

Objective：To compare the efficacy and practicability of Holmium laser lithotripsy with extracorporeal shock wave lithotripsy for acute ureteral calculi. Method.-109 acute ureteral calculi were performed with holmium： YAG laser lithotripsy; while 763 patients were performed with extraeorporeal shock wave lithotripsy. The treatment duration and stone -- free rate of were compared between the two groups. Result： The mean operation duration with extracorporeal shock wave lithotripsy for acute ureteral calculi（16min） was shorter than that with holmium laser lithotripsy（27min） （ P 〈0.05）. The stone- free rate of first operation with holmium： YAG laser lithotripsy and with extracorporeal shock wave lithotripsy was 90.8% and 82.4%, respectively （ P 〈0.05）. Conclusion：Extracorporeal shock wave lithotripsy is simple, effective. It is indicated as the first choice in the treatment of simple acute ureteral calculi. The stone- free rate of first operation with holmium laser lithotripsy is higher than with extracorporeal shock wave lithotripsy. It is indicated as the first choice in the treatment of complexity acute ureteral calculi.
