不同抗生素在不干扰胰腺手术治疗早期重症胰腺炎中的应用研究

目的探讨不同抗生素在不干扰胰腺手术治疗早期重症胰腺炎（SAP）的治疗效果。方法46例SAP患者根据病情选择相应手术和抗生素,观察患者术后情况和药物不良反应。结果手术后46例患者生命体征平稳,无1例出现发热,无1例出现腹腔内感染。第1阶段中有5例（33.3%）使用庆大霉素后7～9d,在尿常规检查中发现蛋白和少量管型,停药改用氨卞西林;第2阶段1例（6.7%）因长期使用氯霉素后出现精神症状,停药后缓解;第3阶段无不良反应发生,3个阶段不良反应发生率成递减趋势,各阶段差异均有统计学意义（P〈0.05）。结论在不干扰胰腺手术治疗SAP时,合理选用抗生素可以明显减少SAP患者的腹腔内感染及其他药物不良反应,缩短手术后患者的住院时间。

Apply research different antibiotics do not interfere with pancreatic surgery in the treatment of early severe acute pancreatitis

Objective To study the antibiotic does not interfere with the pancreas in surgical treatment of early severe acute pancreatitis（SAP） of the therapeutic effect.Methods 46 cases of SAP in patients with surgical methods according to the disease and antibiotics,observed the situation after operation and adverse drug reactions.Results 46 patients with post-operative vital signs were stable,non-one cases of fever,non-one cases of intra-abdominal infections occurred.Other drug reactions：the first stage in 5 cases（33.3%） the use of gentamicin after 79day,urine routine examination were found in the protein and a small amount of tube type,stopping to switch to ampicillin;2 phase 1 case（6.7%）due to prolonged use of chloramphenicol after psychiatric symptoms after stopping ease;3 phase no adverse reactions occur,three stages of the incidence of adverse reactions into a decline,are statistical differences between the various stages of significance（P〈0.05）.Conclusion Without interfering with the surgical treatment of pancreatic SAP,the rational use of antibiotics can significantly reduce the SAP in patients with intra-abdominal infection and other adverse drug reactions,reduce post-operative patient＇s hospital stay.
