改良冷热中药交替浸泡疗法对脑卒中后肩手综合征的疗效观察

摘    要:目的:观察改良冷热中药交替浸泡疗法对脑卒中后肩手综合征的临床疗效。方法:将80例脑卒中的患者随机分为治疗组与对照组,两组均接受常规治疗及康复训练,对照组进行冷热交替浸泡治疗,治疗组进行改良的冷热中药交替浸泡的疗法。1次/d,20min/次,两组疗程均为4周。两组患者于治疗前后分别进行视觉模拟评分(VAS)、简化Fugl-Meyer运动功能评测、表面肌电测试。结果:对照组与治疗组在治疗前的各项观察指标相比较均无统计学意义(P〉0.05)。治疗4周后,治疗组上肢及手的运动功能改善较对照组明显(P〈0.05),治疗组腕手部疼痛的视觉模拟评分较对照组改善明显(P〈0.05),治疗组表面肌电旋前圆肌肌力较对照组增强程度大(P〈0.05)。结论:改良冷热中药交替浸泡对脑卒中后肩手综合征的疗效明显优于仅进行冷热交替浸泡的治疗方法。新型中西医结合的康复治疗方法对于脑卒中后肩手综合征的康复疗效更优。

Clinical observation on treating shoulder-hand syndrome after stroke in TCM
Abstract:Objective: To observe clinical effects of TCM medicine on shoulder-hand syndrome after stroke. Methods: 80 patients were randomly divided into treatment group and control group; all patients were given routine treatment and rehabilitation training. The control group was given alternating hot and cold soaking more, while treatment group was given alternating hot and cold soaking in TCM medicine more. Both groups were lasted for 4 weeks, 1 time a day, each for 20 minutes. Two groups were respectively evaluated by pain visual analogue score (VAS), simplified Fugl-Meyer motor function score (SFMA), surface electromyography (sEMG) before and after treatment. Results: The observation indexes in two groups before treatment had no statistically significant difference (P〉0.05). 4 weeks later, the upper extremity and hand movement function and the spontaneous pain VAS score in wrist hand were improved significantly compared with control group, (P〈0.05). The muscle strength of teretipronator assessed by sEMG in treatment group were enhance compared with control group (P〈0.05). Conclusion: Clinical effects in the treatment group were better, and are effective on shoulder-hand syndrome after stroke.
