西洛他唑与阿司匹林联合应用在神经介入治疗中的抗血栓作用

目的探讨抗血小板药物阿司匹林与西洛他唑联合应用在神经介入术中的抗血栓作用。方法19例颈动脉狭窄行支架置入术的病人随机分为西洛他唑组（n=13）和氯吡格雷组（n=6）。17例未破裂动脉瘤行栓塞术的病人随机分为西洛他唑组（n=12）和氯吡格雷组（n=5）。术前西洛他唑组口服西洛他唑和阿司匹林，氯吡格雷组口服氯吡格雷和阿司匹林。术后48h行弥散加权成像（DWI）检查，评估缺血性病灶出现及术后两周内脑缺血事件发生的情况。结果在颈动脉狭窄支架置入术及动脉瘤弹簧圈栓塞术中，西洛他唑组与氯吡格雷组出现缺血病灶及脑缺血事件的差异均无统计学意义（P〉0．05）。结论联合应用西洛他唑与阿司匹林，可有效降低颈动脉支架置入术及动脉瘤栓塞术中血栓形成，其效果并不亚于氯吡格雷。

Effectiveness of combination of cilostazol and aspirin in preventing thrombosis after interventional neuroradiology procedures

Objective To investigate the effectiveness of combination of aspirin and cilostazol as antiplatelet agents in preventing thrombosis after interventional neuroradiology. Methods Nineteen patients with carotid stenosis who underwent carotid stenting were randomly divided into cilostazol group （n=13） and clopidogrel group （n=6）. Seventeen patients with intracranial aneurysms who underwent interventional embolization were also randomly divided into cilostazol group （n=12） and clopidogrel group （n=5）. Patients in the cilostazol group were treated with cilostazol and aspirin, and clopidogrel group with clopidogrel and aspirin before operation. All of the patients underwent diffusion-weighted MR imaging （DWI） examination within 48 h after operation, postoperative new cerebral ischemic lesions occurred and ischemic event was evaluated 14 days later. Results During the carotid stenting and interventional coil embolization, the difference between eilostazol and clopidogrel groups was not statistically significant in the occurrence of the detected new cerebral ischemic and ischemic event （P〉0.05）. Conclusion The combination of aspirin and cilostazol is effective in preventing thrombosis after carotid artery stenting and interventional coil embolization for treatment ofun-ruptured aneurysms, which is as same as clopidogrel.
