钽棒植入联合骨髓基质干细胞动脉灌注治疗股骨头坏死的疗效观察与护理

目的 观察钽棒植入联合经股动脉骨髓基质干细胞灌注术治疗成人非创伤性股骨头坏死疗效并报告护理体会.方法 选取成人非创伤性股骨头坏死患者30例（40髋）,采用多孔钽棒植入联合经股动脉骨髓基质干细胞灌注术治疗,术前术后予以适宜护理措施,通过临床指标、影像学指标及髋关节Harris评分对患者进行临床疗效评价.结果 28例（38髋）患者均获得随访,32髋关节疼痛有不同程度的缓解,20例行走间距延长,17例关节功能改善.数字减影血管造影（DSA）示股骨头血供较治疗前明显改善,X线摄片示27髋股骨头坏死区域未进展,MRI扫描示15髋股骨头坏死区可见部分骨质病变获得修复.随访髋关节Harris功能评分满意.结论 钽棒植入联合股动脉骨髓基质干细胞灌注治疗成人非创伤性股骨头坏死近期疗效满意.实施合理的护理措施,可以让患者更好地配合治疗,同时减少相关并发症发生,促进患者的康复.

The Effect of Tantalum Joint Bone Marrow Stromal Stem Cells Implanted Artery Perfusion on Non-Traumatic Femoral Head Necrosis

objective： To assess the clinical efficiency of tantalum joint bone marrow stromal stem cells implant- ed artery perfusion on non-traumatic femoral head necrosis as well as the nursing experiences. Methods： patients of non- traumatic femoral head necrosis were treated by tantalum and bone marrow stromal stem cells implanted artery perfusion. The clinical symptoms and signs,imaging,the Harris score were compared to evaluate the clinical effect. Result： 28 cases （38 hips） patients were followed up. 32 hips had different degrees of pain relief,and 20 patients＇ routine walk distance extend, 17 cases of joint function improved. DSA showed that blood supply of femoral head was obviously improved. The X-ray showed 27 hip femoral head necrosis area were not progressed, and MRI showed 15 hip femoral head necrosis area visible parts for repairing bone lesions. Harris score showed satisfactory result. Conclusion： The clinical efficiency of tantalum,bone marrow stromal stem cells implanted artery perfusion with nursing on non- traumatic femoral head necrosis is effective,worthy of promotion and appli- cation for the future.
