改良横切口在内侧乳腺肿瘤手术的应用

目的在横切口（Stewart切口）的基础上对内侧乳腺癌患者手术切口进行改良，成为逗号形横手术切口，减少术后并发症。方法对80例可以行横切口的内侧乳腺癌手术（非保乳）病人，采用横切口（对照组）和逗号形横手术切口（实验组）进行手术治疗。术后观察病人皮瓣坏死和肩关节活动恢复等指标。结果采用逗号形横手术切口的内侧乳腺癌病人，切口皮肤坏死率较低，肩关节功能恢复较好。结论逗号形横手术切口应用滑行皮瓣原理，较好地利用了外侧过多的皮肤，解决内侧乳腺癌可用皮肤较少的问题，减少了术后并发症。

Application of modified transverse incision on wall breast cancer operation

Objective To reduce the complications for modifying the steward incision to comma-shape steward incision on wall breast cancer patients. Methods Totally 80 eases with wall breast cancer were randomly divided into two groups transverse incision group（ control group） and comma-shape steward incision group（ experiment group）. The skin flap necrosis and the recovery of shoulder joint activity were observded after operation. Results The rate of skin flap necrosis and enhanced shoulder joint function recovery in experiment group were obviously lower than that of control group. Conclusion Sliding flap principle were used in comma-shape steward incision operation, which reasonablely used the lateral spare skin to solve the problem of the wall skin insuf. Less complications occured.
