鼻内应用肾上腺皮质激素治疗鼻息肉后ICAM-1及sICAM-1的表达及意义

摘    要:目的:检测鼻内应用肾上腺皮质激素(激素)治疗鼻息肉前后鼻息肉组织中的细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)和鼻分泌物中的可溶性细胞间黏附分子-1(soluble intercellular adhesion molecule-1,sICAM-1)表达水平的变化,探讨ICAM-1、sICAM-1两者间关系及其在鼻息肉发展过程中的作用及鼻内应用激素治疗鼻息肉的可能机制.方法:选择19例未用药的鼻息肉患者(未用药组)、14例经鼻内应用激素(氟替卡松)治疗后的鼻息肉患者(激素治疗组)、12名正常人(正常对照组),取其鼻息肉或鼻黏膜组织及鼻分泌物,用ELISA法检测其鼻息肉或鼻黏膜组织匀浆中ICAM-1和鼻分泌物中sICAM-1的含量并作比较.结果:未用药组鼻息肉组织匀浆中ICAM-1的含量及鼻分泌物中sICAM-1的含量,较激素治疗组及正常对照组为高(均为P<0.01),而激素治疗组与正常对照组鼻黏膜组织匀浆中ICAM-1的含量和鼻分泌物中sICAM-1的含量比较差异无统计学意义(P>0.05).未用药组鼻息肉组织匀浆中ICAM-1及鼻分泌物sICAM-1含量呈线性相关(r=0.843,P=0.000).结论:鼻内应用激素的治疗效果可能与其抑制ICAM-1的表达有关.在未受药物干扰的情况下,鼻分泌物中 sICAM-1的含量也能很好的反映鼻息肉组织中ICAM-1的表达情况.

Abstract:Objective:To study the expression of intercellular adhesion molecule-1 (ICAM-1 ) in nasal polyps and soluble ICAM-1 (sICAM-1) in nasal secretion and to study the relationship between ICAM-1 and sICAM-1 as well as their roles in the development of nasal polyps, and to explore the mechanism of nasal polyps treated by glucocorticoid. Methods: Concentration of ICAM-1 in nasal polyps tissue and sICAM-1 in nasal secretion were measured by ELISA and compared among 19 patients with nasal polyps without treatment, 14 with nasal polyps treated by flixonase and 12 healthy controls. Results: Concentrations of ICAM-1 in tissue and sICAM-1 in nasal secretion of patients without treatment were significantly higher than those of patients treated by flixonase and in normal controls (P<0. 01, P <0. 01). The concentrations of ICAM-1 in tissue and sICAM-1 in nasal secretion of patients treated by flixonase were similar to those of normal controls ( P > 0. 05 ). ICAM-1 in tissue was correlated with sICAM-1 in nasal secretion of patients without treatment (r = 0. 843 , P = 0. 000). Conclusion: The effect of flixonase treatment is probably related to its inhibition on the expression of ICAM-1. sICAM-1 in nasal secretion can reflect the content of ICAM-1 in nasal polyps in patients without medical intervention.
