男性2型糖尿病患者血清维生素D水平与周围神经病变的相关性

摘    要:目的:探讨男性2型糖尿病( Type 2 diabetes mellitus,T2DM)患者血清中25羟维生素D(25-hy-droxyvitamin D,25(OH)D)水平及其与周围神经病(diabetic peripheral neuropathy,DPN)的相关性。方法选取遵义医学院附属医院内分泌科住院部的男性2型糖尿病患者230例,根据有无周围神经病变分为非周围神经病变组(NDPN,n=107)和糖尿病周围神经病组(DPN,n=123),同时选取我院体检中心的健康男性50例,作为正常对照组(NC,n=50),分析三组间血清25(OH)D、空腹静脉血糖(FBG)、空腹胰岛素( FIns)、血脂谱[甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)及高密度脂蛋白(HDL-C)]水平;将三组间25(OH)D的缺乏程度做比较。结果(1)DPN组血清25(OH)D水平较NDPN组明显降低(P<0.05),NDPN组血清25(OH)D水平较NC组明显降低(P<0.05);(2)Pearson相关分析显示血清25(OH)D水平与FIns、HDL-C水平呈正相关(P<0.05),与HbA1c水平呈负相关(P<0.05)。结论血清25(OH)D的缺乏与T2DM和DPN的发生密切相关。

The Correlation between the Level of Vitamin D and Diabetic Peripheral Neuropathy in Male Patients with Type 2 Diabetes Mellitus
Abstract:Objective To explore the correlation between the level of 25-hydroxyvitamin D (25(OH)D) and diabetic peripheral neu-ropathy (DPN) in male patients with type 2 diabetes mellitus (T2DM). Methods 230 male cases with T2DM hospitalized in Department of Endocrinology, the Affiliated Hospital to Zunyi Medical College were selected and divided into 2 groups:NDPN group (cases with no DPN, n=107) and DPN group (cases with DPN, n=123);50 normal cases were selected to be the control group;analysis was made to 25(OH)D, fasting blood glucose (FBG), fasting insulin (FINS), total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL-C) and high density lipoprotein (HDL-C) of the cases in the 3 groups and a comparison of 25(OH)D deficiency was made among the 3 groups. Results The level of 25(OH)D of the cases in DPN group was much lower than that of the cases in NDPN group (P<0.05) while the level of 25(OH)D of the cases in NDPN group was much lower than that in control group(P<0.05);Pearson Correlation Analy-sis showed that there existed a positive correlation between the level of 25(OH)D and the level of Fins and HDL-C (P<0.05) while a neg-ative correlation between the level of 25(OH)D and the level of HbAlc (P<0.05). Conclusions 25(OH)D deficiency is closely correlated to the occurrence of T2DM and DPN.
