妊娠晚期潜在危重孕妇预警指标体系的构建

目的 建立科学、合理的妊娠晚期潜在危重孕妇预警指标体系,为早期识别危重孕妇提供理论指导。 方法 采用德尔菲法对45名专家进行两轮专家咨询,确定妊娠晚期潜在危重孕妇预警指标;采用层次分析法确定各指标权重。 结果 最终确定的预警指标体系包括5项一级指标及28项二级指标。专家函询问卷的有效回收率分别为97.78%和95.45%(＞90%);专家权威系数(Cr)分别为0.927和0.936;两轮专家函询专家意见协调系数分别为0.158和0.297,经过两轮专家函询后指标的变异系数范围为0.07~0.20。5项一级指标及权重分别为一般情况(0.138 4)、症状(0.095 4)、体征(0.183 1)、检查(0.340 9)和检验(0.242 2)。 结论 专家咨询的可信度高,构建的预警指标体系可以全面反应孕晚期孕妇的健康状况,为孕晚期孕妇的病情评估提供依据,有助于识别潜在危重孕妇。

Construction of early warning index system for potential critical pregnant women in late pregnancy

Objective To establish a scientific and reasonable early warning index system for potentially critical pregnant women in late pregnancy, so as to provide a theoretical guidance for early identification of critical pregnant women. Methods Forty-five experts were consulted using the Delphi method to determine the early warning indexes of potentially critical pregnant women in the late pregnancy, and the weight of each index was determined using the analytic hierarchy process (AHP). Results The final warning indicator system included 5 first-level indicators and 28 second-level indicators. The effectiveness recovery rate of the initial questionnaire was 97.78% and 95.45% respectively (> 90%), with the Cr of 0.927 and 0.936. The expert opinion coordination coefficient of the two rounds of expert consultation were 0.154 and 0.157, and the variation coefficient of index after two rounds of expert consultation was between 0.07 and 0.20. The 5 first-level indexes and their weights were general situation (0.138 4), symptoms (0.095 4), signs (0.183 1), inspect (0.340 9) and test (0.242 2). Conclusion The experts consultation is of high reliability. The early warning index system constructed can reflect the health status of pregnant women in late pregnancy, provide the basis for assessing their condition, and help to identify potentially critical pregnant women.
