体外循环心脏停跳和不停跳心内直视手术对机体免疫功能的影响

摘    要:目的观察体外循环心脏停跳和不停跳心内直视手术对机体免疫功能的影响。方法60例风湿性二尖瓣膜病变患者,随机分为实验组(不停跳组,40例)和对照组(停跳组,20例),分别于10个时点抽取静脉血,测定T细胞亚群、NK细胞以及IgA、IgG、IgM。结果两组CD3+、CD4+、NK细胞以及IgA、IgG、IgM在转机后下降,CD8+上升,两组比较P≤0.05,实验组CD3+、CD4+、CD8+、NK细胞以及IgA、IgG、IgM恢复较对照组快(P均〈0.05)。结论体外循环不停跳心内直视手术对机体免疫功能影响较心脏停跳者轻。

Abstract:Objective To study the different of the effect on immune function in beating heart surgery and cardioplegia surgery during cardiopulmonary bypass (CPB). Methods Sixty patients suffered from rheumatic mitral disease were randomly divided into two groups: the trial group ( beating heart surgery,40cases)and the wlfile control group (eardioplegia surgery ,20cases ), Venous blood samples were collected at ten different time intervals to examine the levels of CD3+ , CD+, CD+ , NK cell and IgA,IgG,IgM. Results The Results showed that CD+, CD+ , NK cell and IgA,IgG,IgM in both groups decreased significantly before CPB, CDs in both groups increased significantly before CPB, the level of CD+ ,CD+ ,CD+, NK cell and IgA,IgG,IgM need less time to recover in trial group during CPB to normal level than that in while control group. Conlusion The effect on immune function in beating heart surgery are less than cardioplegia surgery during cardiopulmonary bypass.
