摘    要:目的评价99Tcm-MDP全身骨显像对多发性骨髓瘤的诊断价值.方法对15例经实验室检查、临床骨髓穿刺和手术病理证实的多发性骨髓瘤进行回顾性分析,15例做了SPECT全身骨显像和X线检查,其中7例做了CT扫描.结果多发性骨髓瘤病变部位以肋骨、腰椎、胸椎、头颅骨为最多,其次为肩胛骨、骨盆、四肢骨、胸骨和锁骨等,主要表现为病变部位呈异常放射性浓聚灶和多发性浓聚合并放射性稀疏减淡或缺损区,CT或X线检查主要表现为溶骨性破坏.结论 SPECT全身骨显像结合X线、CT检查不失为诊断骨髓瘤的一种辅助方法.SPECT骨显像对多发性骨髓瘤病人化疗疗效的监测和预后评价具有一定临床价值.

Abstract:Objective To investigate the clinical value of ^99Tc^m MDP bone scan in multiple myeloma. Methods A retrospective analysis of 15 patients with multiple myeloma verified by laboratory examination and histopathologically finding on marrow biopsy specimens was performed. All of the patients underwent SPECT whole body scan with ^99 Tc^m -MDP and X-ray examination, and 7 of them underwent also X-CT scan. Results Foci of 15 patients with multiple myeloma located mainly in rib, spine, lumbar and skull, next in scapula, pelvis, limbs, sternum and calvicle. The SPECT whole body bone scintigraphy showed multiple abnormal increasing or combined with increasing and decreasing or defect of radioactive uptake in the lesions. The findings of X ray or CT scan indicated osteolytic destruction. Conclusion SPECT combined with conventional X-ray and CT scan might be the preferred imaging examination method for multiple myeloma. SPECT bone imaging has important values in monitoring chemotherapeutic efficacy for multiple myeloma and in evaluating its prognosis prediction.
