前置胎盘的期待治疗效果观察及注意事项（附80例报告）

目的观察前置胎盘期待疗法的临床效果及注意事项。方法前置胎盘80例应用硫酸镁或舒喘灵抑制宫缩、地塞米松促胎肺成熟、预防感染等期待治疗。结果终止妊娠时孕周平均延长（1．5±0．2）周；产后出血（369．3±72．3）ml，产褥感染2例；新生儿体质量（3．21±0．35）Kg、Apgar评分（9．1±0．6）、新生儿呼吸窘迫综合征（KDS）3例、死亡2例。2例部分低置胎盘转为正常获得自然分娩。结论期待治疗前置胎盘延长孕龄、改善存活儿的预后、降低围生儿死率，具有可行性。实施中应注意．在严密监测下，期待治疗过程中出现胎儿窘迫征象者，孕妇反复发生多量出血甚至休克者，应随时终止妊娠.

Clinical Effect of Expectant Treatment of Placenta Previa and Considerations

Objective To observe the clinical effect of expectant treatment of placenta previa and precautions. Methods 80 cases of placenta previa were treated with expectant therapies such as magnesium sulfate or salbutamol to inhibit uterine contractions, dexamethasone to promote fetal hmg maturity and to prevent infection. Results At termination of pregnancy, the average gestational weeks extended to （1.5± 0.2） weeks; postpartum hemorrhage （369.3± 72.3） ml, puerperal infection in 2 cases; neonatal body mass （3.21 ±0.35） Kg, Apgar scores （9.1± 0.6）, neonatal respiratory distress syndrome （RDS） 3 cases, 2 deaths. 2 cases of partly low placenta were normal to natural childbirth. Conclusion Expectant treatment of placenta previa can extend the gestational age, improve the prognosis of child survival and reduce perinatal child death rate, which is feasible. By closely monitoring, pregnant women who have signs of fetal distress, recurrent bleeding and even shock in the course of expectant treatment, should terminate pregnancy at any time.
