弥漫大B细胞淋巴瘤中CD27的表达及临床意义

目的探讨弥漫大B细胞淋巴瘤(diffuse large B cell lymphoma, DLBCL)中CD27的表达及其临床意义。方法采用免疫组化EnVision法检测143例DLBCL组织中CD27蛋白的表达;应用FISH技术检测DLBCL组织中MYC、BCL-2、BCL-6基因重排情况。结果 143例DLBCL中,CD27蛋白阳性者46例,阳性率为32.2%。CD27阳性组中BCL-2重排阳性率(17.4%)明显高于CD27阴性组(4.1%),差异有统计学意义(P<0.01)。CD27阳性组病死率(30.4%)明显高于CD27阴性组(15.5%),差异有统计学意义(χ²=4.326,P=0.038)。CD27阳性者与阴性者的Kaplan-Meier生存曲线差异有显著性(χ²=4.485,P=0.034),阳性组生存期较短。结论 CD27高表达与DLBCL预后密切相关,可作为临床预后评价的指标之一。

Expression and clinical significance of CD27 in diffuse large B-cell lymphoma

Purpose To investigate the expression of CD27 in diffuse large B-cell lymphoma(DLBCL)and its clinical significance.Methods Immunohistochemical of EnVision staining was used to detect the expression of CD27 protein in tumor tissues of 143 DLBCL patients.Fluorescence in situ hybridization(FISH)was used to detect the rearrangement of MYC,BCL-2,and BCL-6 genes in tumor tissues.Results 46 of the 143 DLBCL patients were positive for CD27 protein,and the positive rate of CD27 expression was 32.2%.The proportion of rearrangement of BCL-2 in the CD27-positive group(17.4%)was significantly(P<0.01)higher than in the CD27-negative group(4.1%)(P<0.01).The proportion of death in the CD27-positive group(30.4%)was significantly higher than in the CD27-negative group(15.5%),and the difference was statistically significant(χ^2=4.326,P=0.038).The Kaplan-Meier survival curve showed CD27 positive group and negative group were significantly different(χ^2=4.485,P=0.034).Conclusion In DLBCL patients,higher expression level of CD27 is closely related with prognosis of DLBCL.It can be used as one of the indicators of clinical prognosis evaluation.
