组织多普勒超声检测单心室心功能

目的 探讨组织多普勒超声测定的心肌工作指数在单心室心功能评价中的价值。方法 以47例功能性单心室患儿和28例正常儿童为研究对象。应用组织多普勒成像技术测定心肌工作指数（MPITDI），并与血流多普勒测定的心肌工作指数（MPID）、心导管测定的心室压力变化率峰值（dp／dtmax）进行对比分析。结果 在单心室组和正常儿童组中，MPITDI与MPID之间均具有良好的相关性（分别为r=0．912，r=0．944）。单心室患儿的MPITDI与dp／dtmax。呈显著负相关（r=-0．777）。与正常儿童相比，单心室患儿的MPITDI、等容收缩时间、等容舒张时间显著增高。结论 组织多普勒超声测定的心肌工作指数是一项测量简便、重复性好的检测单心室心功能指标。单心室患者的心功能较正常儿童差。

Value of myocardial performance index derived by tissue Doppler imaging for evaluating ventricular function of functionally single ventricle

Objective To explore the value of tissue Doppler imaging(TDI) derived myocardial performance index(MPI_ TDI ) for assessing ventricular function in patients with functionally single ventricle. Methods Subjects consisted of 47 patients with functionally single ventricle and 28 normal children without heart disease. MPI_ TDI was measured by TDI, and compared with pulsed Doppler derived MPI_D and dp/dt_ max obtained by cardiac catheterization. Results TDI derived MPI_ TDI correlated well with pulsed Doppler derived MPI_D in patients with functionally single ventricle and normal children( r = 0.912 , 0.944 , respectively). MPI_ TDI had a good correlation with dp/dt_ max ( r = -0.777 ) measured through catheterization in patients with functionally single ventricle. Compared with normal children, patients with functionally single ventricle had significantly increased MPI_ TDI and longer isovolumetric contraction and relaxation time.Conclusions TDI derived MPI_ TDI is a simple, reproducible index for assessing ventricular performance in patients with functionally single ventricle.Compared with normal children,ventricular function is impaired in patients with functionally single ventricle.
