左房增大及P波离散度与阵发性房颤相关性探讨

目的探讨左房增大对阵发性房颤的影响及P波离散度变化的意义。方法根据超声心动图结果将108例患者分成左房增大及心房内径正常组。用24h动态心电图记录所有患者阵发性房颤发生情况，同时记录窦性心律时12导心电图P波离散度（PD），并探讨他们之间的相关性。结果左房扩大的患者，其PD和Pmax均较心房内径正常者显著增大，阵发性房颤的发生率也明显增加。‘结论左房增大是引起心房电生理紊乱、产生房颤的重要因素之一。

Discussion on the Relationship among Left Atrial Enlargement and P Wave Dispersion and Paroxysmal Atrial Fibrillation

Objective To investigate the impact of left atrial enlargement on paroxysmal atrial fibrillation and analyze the signifi- cance of changes of P wave dispersion. Methods The left atrial diameters of 108 patients with paroxysmal atrial fibrillation were measured by eehocardiography. According to the results of echocardiography, the patients were divided into two groups, Group left atrial enlargement and Group normal left atrial. The incidence of paroxysmal atrial fibrillation in 108 patients was recorded by 24- hour ambulatory electrocardiogram. And the P wave dispersion （PD）of a sinus rhythm selected from the 12-lead electrocardiogram （ECG） was recorded. And the relationship of them was discussed. Results PD and Pmax of patients with left atrial enlargement were significantly larger than those of patients with normal left atrial, and the incidence of paroxysmal atrial fibrillation was obvi- ously higher than that of patients with normal left atrial. Conclusion Left atrial enlargement was an important factor in producing atrial electrophysiologieal disorder and atrial fibrillation.
