摘    要:目的探讨抗精子抗体(ASAb)对IVF—ET技术卵子成熟度、受精率、卵裂率及胚胎质量的影响。方法分析36例血清ASA阳性妇女,19例精子表面ASA阳性男性患者IVF—ET周期受精率、卵裂率及胚胎质量与对照组的区别。结果女性血清中ASAbrgM为多,男性精子表面IgA联合IgG为主,两种情况均可降低卵子的受精率和卵裂率,但对卵子成熟度和胚胎质量无影响;行IVF—ET的ASAb阳性妇女受精率和卵裂率明显低于对照组,行ICSI的ASAb阳性妇女各参数与对照组无明显差异。结论女性血清中IgM和男性精子表面IgA及kG能降低体外受精的受精率及卵裂率;对选择体外受精助孕的夫妇当女性血清中IgM阳性,男性精子表面IgA和kG同时阳性时ICSI是优先选择的助孕方式。

Abstract:Objective: To discuss the effect of ASAb on the mature of oocyte, fertilization rate, cleavage rate and quality of em- bryo. Methods: 36 female who are serum positive of ASAb and 19 male who are sperm surface positive of ASAb as the study group, compared the mature of oocyte, fertilization rate~ cleavage rate and quality of embryo with control group who are negative of ASAb. Results : The IgM is more than other substyle in the female serum, the main substyle is IgA combining IgG on the surface of sperm, the fertilization rate and cleavage rate are low in such people, but the mature of oocyte and embryo's quality are not; compared with the IVF - ET control group, the fertilization rate and cleavage rate are low significantly in IVF - ET study group, and all the pa- rameters of ICSI study group are similar to the ICSI control study. Conclusion: The IgM of ASAb in women's serum and IgG combind with IgA on the surface of sperm can greatly decrease the fertilization rate and cleavage rate ; ICSI is the first choice for couples who se- lect assisted reproduction technology.
