二孔法腹腔镜良性卵巢囊肿剥除术的临床分析

目的:总结二孔法腹腔镜卵巢良性肿瘤剥除术的适应症、手术技巧.方法:对我院进行的29例二孔法腹腔镜良性卵巢囊肿手术进行分析.结果:术后病理诊断巧克力囊肿8例,单纯滤泡囊肿10例,输卵管系膜囊肿(卵巢冠囊肿11例);手术均顺利完整剥除囊肿,保留了卵巢,手术历时平均65min;平均失血量约15ml;术中延长切口最多5mm.术后6h进流质饮食,嘱下床活动,无须术后镇痛,停留尿管12～24h,术后常规抗炎治疗3d,无伤口感染或腹膜炎等并发症;平均住院时间4～6d.术后随访患者满意率达100%,暂无复发病例.结论:严格掌握二孔法腹腔镜卵巢良性囊肿剥除术的适应症及手术技巧,能达到三孔法腹腔镜及开腹手术更好的治疗效果.

Clinical Analysis for Operation of Stripping Benign Ovarian Cyst Using Double-drilled Laparoscopic Technique

Objective: To summarize the indications and the skills of Laparoscopic surgery with double-drilled technique on treatment of benign ovarian cyst.Method: 29 operation cases of benign ovarian cystic tumors using doubledrilled laparoscopic technique are analyzed.Result: Among the pathological diagnoses of these 29 cases,there were 8 chocolate cysts,10 simple follicle cysts and 11 parovarian cysts.All patients'cyst were stripped completely and their ovary were reserved in operation.On the average these surgical operations lasted 65 minutes,losed 15 ml amount of blood ,and no more 5 mm incision extended.After operation,6 hours later these patients began to eat liquid food and walk about bed,no pain-killer used,catheter reserved for 12-24 hours and some certain antibiotics used for 3 days.No infection occurred;Average hospitalization time is 4-6 days.All patients are satisfied with the effect of the operation and no illnesses recurred up to now.Conclusion: If the indications and skills are mastered well,operation of stripping benign ovarian cyst using double-drilled laparoscopic technique can get better medical effect than using tri-drilled laparoscopic technique and laparotomy.
