乳腺实性乳头状癌全数字化乳腺摄影表现与病理对照

目的:探讨乳腺实性乳头状癌(solid papillary carcinoma,SPC)的全数字化乳腺摄影(full-field digital mammography,FFDM)表现及临床病理特征,提高对该病的诊治水平。方法:回顾性分析经手术病理证实的15例乳腺SPC患者的资料,分析其临床病史、FFDM表现及病理特征,并对SPC的FFDM征象进行二项式检验。结果:15例SPC患者中,1例合并高级别导管内癌,1例合并黏液腺癌。13例患者FFDM图像上发现肿块,其中12例表现为高密度影;9例病灶边缘清晰,4例边缘模糊;13例均表现为边缘浅小分叶;5例病灶内见钙化。1例表现为不对称性致密影,1例未见明显肿块。二项式检验结果显示,边缘浅小分叶、高密度差异有统计学意义(P0.05),其他征象差异无统计学意义。结论:乳腺SPC的FFDM表现为边缘浅小分叶、高密度等,大多数病灶边缘清晰,且与病理特征相关。

Solid papillary carcinoma of breast:imaging features of full-field digital mammography correlated with pathological findings

Objective To investigate the appearance of solid papillary carcinoma(SPC) of breast on full-field digital mammography(FFDM) correlated with clinical and pathological findings.Materials and Methods Fifteen patients with breast SPC diagnosed by pathology were enrolled.The correlation among FFDM imaging findings,clinical histories and pathological characteristics were analyed.Binomial test was applied to evaluate the features of SPC on FFDM.Results All patients were diagnosed SPC by pathology,and 1 case was associated with high grade ductal carcinoma,while 1 case with mucinous carcinoma.On FFDM,13 cases displayed mass with high density(12 lesions),clear(9 lesions)or?blurred(4 lesions)?margin,microlobulated margins(13 lesions) and calcification of lesions(5 lesions).1 case appeared with asymmetric high density.1 case displayed no obvious mass.The binomial test showed us that the differences of high density and microlobulated margin were statistically significant,but there was no significant difference in other signs.Conclusions There are some characteristics on FFDM of SPC,such as microlobulated margins and high density,and most lesions show clear margin.They are all related to pathological features.
